feat(skills): akashi skill polish (tracing recipe, savings framing) - #73

Query-first grounded retrieval against the user's Akashi vault over the mcp-client extension (HTTP + per-runtime bearer token): mandatory source:lines citations with signed provenance, groundedness verdicts surfaced, consent-based write-back following vault conventions, and typed helpers for the ground response shape with tests. Akashi never executes; tool results are treated as data. Published to the Hub as slug akashi; joins the 8 official skills at platinum quality (README, CHANGELOG, LICENSE, types, tests).

Version bump + changelog: the 1.0.0 archive on the Hub predates the tracing recipe and savings framing merged in #73. Republishing as 1.0.1 right after merge.

Follow-up to #71: adds the cross-note tracing recipe (note_context + backlinks composition) to the skill protocol and the per-token savings framing to the README. Matches the current published Hub copy.
