fix(servers): approve a stdio server's command per device before spawning it - #6003
Gate every local stdio spawn where the command is final: applyLocalRuntimeResolution, which the /api/mcp connect and the web-route stdio divert both go through.
Bind the approval to this device and to the exact launch spec (command, args, env names and values, cwd); ask again when it changes.
Ask only on the user's own actions, between connect attempts; auto-connect never prompts.
Inspector only, no backend change.
User story
As a member of a shared project who runs the local or desktop inspector, I want MCPJam to show me the exact command a project STDIO server or a plugin's local component will run, and ask me once on this device before starting it, so that a teammate's edit cannot run code on my machine without my knowledge.
Acceptance criteria
A project stdio server this device never approved is not started. A manual Connect shows the command line, env variable names and working directory; values stay hidden except for variables that make the command load other code (NODE_OPTIONS, LD_PRELOAD, PATH, ...), shown with a warning. Allow records the approval and connects.
A change to the command, args, env (names or values) or cwd asks again, says the command changed, and names the env variables added, changed or removed.
A plugin's local component is gated too. The environment tools pane refuses it with "Use it in a chat to review and approve the command"; a chat turn opens the same dialog, and after Allow the next message runs it.
Cancel leaves the server unstarted: a plugin version whose NODE_OPTIONS loads extra code never ran.
Auto-connect never prompts; the server card shows "Command not approved on this device".
Surfaces that spawn without the client (swarms, eval authoring, the environment tools pane, /api/v1) fail closed with the server's message; /api/v1 answers FORBIDDEN.
What this does
Any workspace member can edit what a project stdio row runs, a plugin component's env included, and the local inspector spawned it on every teammate's machine showing only the server name. The browser never sends the command: the local server re-reads it from Convex at spawn, so the gate sits in applyLocalRuntimeResolution (server/utils/local-server-resolver.ts). It fingerprints the spec as configured, after the secret reveal and before plugin materialization, and compares it with this device's approval in ~/.mcpjam/inspector/stdio-approvals.json (0600, lockfile-serialized, per-variable value hashes so a refusal can name what changed; MCPJAM_STDIO_APPROVALS_FILE relocates it, nothing disables it). A refusal is a 403 STDIO_COMMAND_APPROVAL_REQUIRED carrying the server name and the terms; POST /api/mcp/servers/approve-command re-reads the row and records the approval only if the fingerprint still matches what would run now, or answers 409 with the current terms. The client asks between attempts: a manual connect retries after Allow, and the chat, the only place a plugin's local component starts, opens the dialog on a refused turn and asks the user to send again. Interactive chat turns now tolerate a server that fails (main), so the shared chat turn, which both the web and the local chat routes run, fails when a refused server is waiting for approval instead of quietly running without it; other tolerant callers keep skipping it.
Also: the OAuth profile capture only probes an http row, so it never builds a stdio config outside the gate.
Live pass on 2026-10-27, local inspector from this branch against a dev deployment, signed in, with a real plugin whose local component reports its env. Re-run on 2026-10-28 after merging main, where the local chat moved onto the shared chat turn, against the dev deployment updated to backend main.
Before
After
The component started on any chat that used it, showing only the server name
Chat turn refused, approval dialog with the plugin's command
After Allow and resend, the plugin answered
Plugin version adding NODE_OPTIONS: the dialog shows its value with a warning; Cancel, nothing ran
Plain stdio server: "has changed since you approved it"
Server card: "Command not approved on this device"
Deleted
The plugin-component exemption in applyLocalRuntimeResolution; the gate now runs before materialization for every stdio row.
Reused
createCapabilityMutationLock and persistCapabilityState (server/utils/local-capability.ts), the proper-lockfile pattern of device-consent.ts, readAuthorizedStdioLaunchSpec, parseLocalConnectRequestBody and respondWithLocalRouteError, WebRouteError/describeError, authFetch, the check queue's markManual, and the design-system AlertDialog.
Production signal
None: local and desktop only. A refusal is the local route's 403 STDIO_COMMAND_APPROVAL_REQUIRED; hosted deployments never spawn stdio servers.
Automated decisions
None: no automated decision about users
Tests
Server: approval store (fingerprint, store states and changed names from one read, per-variable hashes, removed names, a file that cannot be read or parsed on approve, 0600), the code-loading values in the terms, the resolver gate (connect and divert paths, revealed env, plugin components refused before materializing, a component with no placeholder pointed at the chat), approve-command (400/401, match, 409 with changedEnvNames, http refused), the chat turn failing on a pending approval and disconnecting, the OAuth profile capture guard, and the v1 mapping.
Client: terms parsing, the approval flow (one dialog and one request for concurrent refusals, re-prompt on 409, failed or unreachable approvals reported without throwing), the chat-error prompt and its call site in useChatSession, the dialog (code-loading warning, escaped control characters, changed and removed variables, double click), the manual-intent handshake in mcp-api (including a job marked manual mid-flight and a failed approval returning the refusal), and manual marks in use-server-state.
After merging current main and the second review round (2026-10-28): 19632 tests across the touched server and client areas pass; typecheck:client and typecheck:server clean; slop:check no rule up; repo hygiene clean.
Live: every criterion above except the same-row env-value change, which the dev deployment cannot store (no KMS for server env); that path is unit-tested. Re-run on the merged code: a plain stdio server and the plugin component were both refused by /api/mcp/chat-v2, asked in the chat, ran nothing on Cancel, and started only after Allow.
Out of scope
Only shared projects: the local server cannot tell whether a project is shared, so every stdio server is gated.
A new plugin version is a new server row, so it asks as a first approval rather than "changed".
Plugin component rows show their internal name (plugin:server:key@rev) in the dialog, as elsewhere in the UI; in chat the error banner repeats the instruction while the dialog is open.
Background reconnects that do not mark themselves automatic (client switch, protocol pin revalidation, the first-run restore) can open the dialog; each follows a user action.
With a caller-supplied queueSignal (eval Start), the caller's queue slot stays held while the dialog is open.
The file lock duplicates withConsentMutationLock in device-consent.ts; per-variable hashes are unsalted SHA-256 in a 0600 file.
Showing every non-secret env value: /web/server/reveal-secrets returns the plugin's declared env and the user's encrypted env as one map, so the inspector cannot tell credentials apart. Only the code-loading variables are shown; the rest needs the backend to name the credential env variables.
No "Review command" action on the card and no way to forget approvals.

…ning it
A project server's command, args and env are editable by every workspace
member (`updateServer` needs workspace role `guest`), and the local
inspector spawned them on this machine with the user's privileges showing
only the server name: from Setup Run, suite reruns, quick runs, case
generation, swarms and the Servers tab alike.
The browser never sends the command; the local server re-reads it from
Convex at spawn. So the gate lives where the stdio config is final,
`applyLocalRuntimeResolution`, which every spawn path goes through (the
/api/mcp connect and the web-route stdio divert). It refuses with 403
`STDIO_COMMAND_APPROVAL_REQUIRED` and the terms it would run (command,
args, env NAMES, cwd, fingerprint) until this device has approved a
SHA-256 of the exact spec, env values included. The approval lives in
`~/.mcpjam/inspector/stdio-approvals.json`, keyed by server id, and a
changed command asks again. Plugin components are exempt: their bundle is
verified at materialization and only a project admin manages them.
`POST /api/mcp/servers/approve-command` records the approval after
re-reading the row; a fingerprint that no longer matches gets the current
terms back (409) instead of binding the click to something else.
On the client, `localConnectionRequest` runs the handshake between
attempts for manual connects only: a dialog shows the command, Allow
records the approval and the connect is retried. Auto-connect never
prompts; the card shows the refusal. Surfaces that spawn without the
client (swarms, eval authoring, environment tools) fail closed with a
message that points at the Servers tab.

…s manual
Found on the first live pass of the per-device stdio command approval:
- `authFetch` attaches the hosted bearer only to listed local routes, and
`/api/mcp/servers/approve-command` was not listed. The dialog opened,
Allow answered 401 and the connect failed with a generic error. Listed
next to `/api/mcp/servers/reconnect`, which re-reads the row the same way.
- The check queue flags a job automatic from a one-shot mark the auto-connect
hook sets, and a mark it left behind (its server was already connected by
the time `ensureServersReady` looked) made the next explicit connect
silent: Save & Connect answered "Connection failed" instead of asking.
User-initiated paths now assert manual intent (`guardedTestConnection`,
`handleReconnect`, and `ensureServersReady` unless the auto-connect hook
passes `automatic: true`), the way the server card already did.

It looks like this branch is under active development. To avoid overwhelming you with review comments due to an influx of new commits, CodeRabbit has automatically paused this review. You can configure this behavior by changing the reviews.auto_review.auto_pause_after_reviewed_commits setting.
The change adds device-local approval records for local project STDIO commands. The server fingerprints launch details, checks approval before resolution, and exposes an endpoint to approve the current command. The client prompts for approval during manual connection attempts, submits approval, and retries; automatic connection attempts do not prompt. Chat errors can also start the approval flow. The change maps the new error at the v1 boundary and limits local OAuth profile capture to HTTP servers.

Thanks, the design holds up well. The gate sits on the one chokepoint that both /api/mcp connects and the web-route stdio divert go through. The approve route binds to what would run now, and the dialog runs between attempts, so it never holds a check-queue slot. I checked the other local spawn paths (conformance, eval replay, harness .mcp.json, agent/docs managers, Electron IPC) and none of them spawns a project stdio row outside applyLocalRuntimeResolution, with one exception noted below.
I ran the touched suites locally on this branch merged with current main: server 155/155 and client 206/206.
Two inline findings: the plugin exemption (a real bypass) and env-only changes being invisible in the dialog. Two more notes that don't fit inline:
1. Merge conflicts with main. The PR is behind main and conflicts in two places:
server/utils/local-server-resolver.ts: an import block. Keep both.
client/src/hooks/use-server-state.ts: main turned handleReconnect into reconnectServerWithResult, which now returns reconnectServerInternal(...). Keep the markManual block above that return. Heads-up: reconnectServerWithResult is also the path agent-driven connects use, so those will now open the approval dialog too. That seems right, since the user is at the UI, but it's worth deciding on purpose.
With those resolved, the suites above pass.
2. Defense in depth: one local spawn outside the gate.server/routes/web/oauth-connections.ts (local branch, ~L77-97) calls authorizeBatchLocal and then withEphemeralClient(toMCPServerConfig(auth, ...)). It never goes through applyLocalRuntimeResolution, and toMCPServerConfig builds a spawnable stdio config when the row is stdio. Today it's only reachable if the batch returns an oauthAccessToken for a stdio row. The backend clears OAuth credentials on an http→stdio flip, so this is a narrow race rather than an open hole. A one-line auth.serverConfig.transportType === "http" guard there would close it for good and make the PR's claim that "every spawn path goes through the gate" literally true.
Review round 1 on #6003.
A plugin component's command and args come from its verified bundle, but
any workspace member can set the row's env (the backend lets plugin rows
take the credential-only path), and the launch copied it into the child.
So the approval gate now runs on every stdio row, before materialization,
over the spec as configured. Plugin local components spawn only through
the web-route divert, with no Connect switch, so the chat opens the same
approval dialog when a turn comes back refused, and the refusal points
plugin components at the chat instead of the Servers tab.
An approval now stores per-variable hashes next to the fingerprint, so a
refusal names the env variables that are new or hold another value, and
the dialog shows them. Values still never leave the server.
Also: the OAuth profile capture never builds a stdio config, the v1
envelope maps STDIO_COMMAND_APPROVAL_REQUIRED to FORBIDDEN (main's
contract test requires a decision for every internal code), the reconciler
tests from main expect the auto-connect batch's { automatic: true }, and
the client approval call reads a non-JSON answer without a bodiless catch.

Fixes from a code review of the approval flow, each verified by hand.
- A double click or a repeated Enter no longer answers the next queued
prompt: the dialog remounts per prompt and ignores a multi-click.
- Control and format characters (a bidi override, a zero-width space) in
the command, args, cwd or env names are shown escaped, so the line the
user approves is the line that runs.
- The refusal carries its own normalized block: the server card read
"Forbidden (403)" with OAuth next steps.
- A connect marked manual mid-flight can ask: the attempt's intent is read
when it returns, not when it starts.
- Prompts for the same server and command share one dialog.
- An approval write that cannot read the file (other than a missing one)
fails instead of rewriting it with only that server.
- The changed-variables line also names variables removed since the last
approval.

…erver
Since main started isolating every server of an interactive chat turn, a
refusal from the stdio approval gate was tolerated like a server that
failed: the turn ran without that server and the chat never saw the 403,
so a plugin's local component could not be approved from anywhere. The
web chat route now fails the turn when a refused server is waiting for
approval, after disconnecting the manager, and the chat opens the
approval dialog as before. Other tolerant callers keep skipping it.
Also drop the ticket id from source comments, per the repo's comment
rules.
Refs: PLB-192
The UI copy review flagged the fallback message for a failed approval as
having no next step. One constant now says to try approving again.
Refs: PLB-192
Line 118 returns early when the process runs as root or on Windows. CI containers often run as root. The test then passes without checking anything. A chmod 000 file is also readable by root.
Make the approvals path a directory instead. readFile then fails with EISDIR, a non-ENOENT error, for every user. This exercises the strict branch in readApprovals. Add mkdir to the node:fs/promises import and drop the unused chmod import.
Proposed fix
- if (process.platform === "win32" || process.getuid?.() === 0) return;
const store = createStdioCommandApprovalStore(() => file);
await store.approve("srv-1", BASE);
- await chmod(file, 0o000);+ const saved = await readFile(file, "utf8");+ await rm(file);+ await mkdir(file); // reading a directory fails with EISDIR for any user
await expect(store.approve("srv-2", BASE)).rejects.toThrow();
- await chmod(file, 0o600);+ await rm(file, { recursive: true });+ await writeFile(file, saved);
Based on learnings: simulate non-ENOENT errors deterministically instead of relying on permission errors, which are unreliable when tests run as root.

Thanks for the review. On the two notes outside the diff:
Merged current main; both conflicts resolved as you described. On reconnectServerWithResult: intended. The agent's ui_connect_server lands there, the dialog waits for the person at the UI, and no agent tool can press its buttons, so an agent can ask to connect a stdio server but cannot approve its command.
Added the transportType === "http" guard to the profile capture in oauth-connections.ts, with a test that a stdio row never reaches withEphemeralClient.
One more from the merge: main's v1 contract test requires a decision for every internal code. /api/v1 evals reach the local divert, so STDIO_COMMAND_APPROVAL_REQUIRED maps to FORBIDDEN at the v1 boundary (envelope.ts), keeping the shared code table byte-identical with the backend.
I also ran a code review over the branch. The findings I verified by hand are fixed in 6bd0393 (double click answering the next prompt, bidi and zero-width characters in the command line, the card reading "Forbidden (403)", a mid-flight manual mark, duplicate prompts, an approval write wiping the file on a read error). The ones left are listed under Out of scope in the description.
…mand-approval
Main moved the web chat route's orchestration into services/chat-turn,
so the pending stdio approval check moves with it: managed-connections.ts
fails the turn right after the manager is created, and the turn's own
resources disconnect it.

Nice work, the gate sits in the right place and the approve route re-reading the row closes the TOCTOU nicely. Two security gaps worth fixing before merge (plugin bundle not in the fingerprint, and changed env values approved blind), plus a few correctness issues and nits inline.

Review round 2 on the stdio command approval:
- The dialog shows the value of NODE_OPTIONS, LD_PRELOAD,
LD_LIBRARY_PATH, DYLD_*, PATH and PYTHONPATH with a warning. Their
value decides what runs and they are never credentials; every other
value stays hidden.
- A plugin row with no ${PLUGIN_ROOT} placeholder is pointed at the chat
too, using the row flag the way materialization does.
- An approvals file that does not parse is moved aside before an
approval rewrites it, instead of being replaced.
- The gate and approve-command build the spec with one function and
check the fingerprint with one pattern; a check reads the file once.
- Refusals of one command share the dialog and the approval request. A
failed approval keeps the refusal and shows a toast instead of
throwing out of the connect.
- The chat parses its error once and hands the details to the prompt,
with a hook test for that call site; the unreadable-file test no
longer skips as root.

Re-reviewed 5e814e4. Everything from my review is addressed:
Bundle hash: agreed, no change needed. I checked the backend: materializePluginServer inserts a fresh servers row per plugin version, and pluginServerComponents is written once, so a new bundle always lands on a new server id with no approval.
Env values: showing code-loading variables with a warning is a good middle ground, and the reason for keeping the rest hidden makes sense.
Plugin component message, unreadable file moved aside, approve no longer throwing, one shared in-flight approval, toStdioLaunchSpec/FINGERPRINT_PATTERN, single parse in handleChatError, single file read: all verified in the diff.
Non-blocking follow-up: CODE_LOADING_ENV_NAME could also cover variables that redirect what npx/uvx download or what a runtime preloads, e.g. NPM_CONFIG_REGISTRY, UV_INDEX_URL, PIP_INDEX_URL, NODE_PATH, PYTHONSTARTUP, PYTHONHOME, JAVA_TOOL_OPTIONS, RUBYOPT, PERL5OPT, BASH_ENV, LD_AUDIT. Careful with NPM_CONFIG_* in general, since some of those are tokens. A second corrupt file also overwrites an earlier .unreadable, which is minor.
Thanks for the re-review. Both non-blocking notes are tracked in PLB-439, the follow-up for showing non-secret env values. Index URLs (UV_INDEX_URL, PIP_INDEX_URL) and NPM_CONFIG_* can carry tokens, so they need the same credential split before their values are shown. The .unreadable overwrite is noted there too.

Any workspace member can edit what a project stdio row runs, a plugin component's
envincluded, and the local inspector spawned it on every teammate's machine showing only the server name. The browser never sends the command: the local server re-reads it from Convex at spawn, so the gate sits inapplyLocalRuntimeResolution(server/utils/local-server-resolver.ts). It fingerprints the spec as configured, after the secret reveal and before plugin materialization, and compares it with this device's approval in~/.mcpjam/inspector/stdio-approvals.json(0600, lockfile-serialized, per-variable value hashes so a refusal can name what changed;MCPJAM_STDIO_APPROVALS_FILErelocates it, nothing disables it). A refusal is a 403STDIO_COMMAND_APPROVAL_REQUIREDcarrying the server name and the terms;POST /api/mcp/servers/approve-commandre-reads the row and records the approval only if the fingerprint still matches what would run now, or answers 409 with the current terms. The client asks between attempts: a manual connect retries after Allow, and the chat, the only place a plugin's local component starts, opens the dialog on a refused turn and asks the user to send again. Interactive chat turns now tolerate a server that fails (main), so the shared chat turn, which both the web and the local chat routes run, fails when a refused server is waiting for approval instead of quietly running without it; other tolerant callers keep skipping it.Also: the OAuth profile capture only probes an http row, so it never builds a stdio config outside the gate.
