fix(chat): stop chats dying when the model calls a tool wrong - #6927
Show the model only a tool's name in search_mcp_tools / load_mcp_tools, never an id.
When the model's tool call is rejected (wrong name or bad input), send the error back to the model and keep the turn going.
User story
As a playground user chatting with an MCPJam model or my own (BYOK) model, with progressive tool discovery on, I want the chat to keep going when the model calls a tool wrong, so that I get an answer instead of "Backend step returned no content" or a chat that just stops.
Acceptance criteria
search_mcp_tools results have a name and no toolId / serverId.
load_mcp_tools loads by names; old chats that sent toolIds still restore and load their tools.
A rejected tool call (unknown name or bad input) goes back to the model as an error result, and the turn takes another step instead of ending.
This also works when the model wrote text before the bad call (before, that chat ended with no error shown).
The step limit still stops a model that keeps calling tools wrong.
Tested end to end with a real model (not done; unit tests only).
What this does
In prod a model was shown each tool with two labels, an id (<serverId>::read_me) and a name (read_me). It loaded the tool by id, then called it by id too. No tool has that name, so the backend rejected the call, and our chat loop treated the empty step as a failure and ended the chat. BYOK hit the same thing but ended silently, because the model wrote some text first. Now the model only ever sees the name. If a call is still rejected, the call and its error are added to history and the loop runs again, which is what the AI SDK's own loop already does. MCPJam models and cloud BYOK both run through this same loop, so one change covers both. Same-name tools from two servers are a separate problem, split out to KES-741; the two revert commits here remove that work from this PR.

scrubUnavailableToolHistoryForBackend's keepHistoryFor hook (built for WebMCP page tools) to keep rejected calls in history.
parseStreamErrorChunkText to give the model the plain error sentence.
lookupToolIdByModelName-style catalog lookups so discovery state stays keyed by internal ids.

Fewer provider_empty_response engine-step failures that name a rejected tool input (mcp.chat-v2.engine-step, Sentry issues like INSPECTOR-ELECTRON-XJ), and fewer "Model tried to call unavailable tool" chat errors.

A rejected call with a provider-unsafe name (like a::b) is stored in history as a__b, in case providers refuse the original; not tested against a real provider.
search_mcp_tools returned each tool with both an id (<serverId>::<tool>) and
a name. A model in prod loaded by id and then called the tool by that id,
which no tool set answers to, and the turn died. Search now returns only the
name, and load_mcp_tools takes names (legacy toolIds still accepted so older
chats keep working). Discovery state stays keyed by internal ids.

…ing the turn
When the backend's AI SDK refused a tool call (unknown name or bad input),
the engine recorded nothing for it: a step with only that call ended the turn
with an empty-step error, and a step that also had text ended silently. MCPJam
models and cloud BYOK both run this loop.
Now the call and an error-text result go into history and the loop continues,
as the AI SDK's own loop does; maxSteps still bounds repeat failures. The call
is marked so the unavailable-tool scrub keeps it, and stored under a
provider-safe name (the error text quotes the original).

Flattening every server's tools into one name-keyed set kept only the last
server's tool when two servers offered the same name; the other was silently
gone. The flatten (getToolsForAiSdk, the lister's isolated path, the model-
executor path) now goes through mergeConnectionToolsets, which keeps the last
server's tool under the plain name, exactly as the model saw before, and
renames the other <tool>__<server slug> with _serverId/_mcpToolName stamped.
Hosted chat passes server display names through so the slug is readable.

Tool merging now assigns unique names to tools with cross-server name collisions, using server display labels where available. Progressive discovery exposes tool names as the model-facing handles and accepts both names and legacy tool IDs when loading tools. Rejected tool inputs are added to chat history with error results, and the stream handler continues the model step loop so the model can retry.
Priority: Normal
Merge Risk: Low · previous review 5dab3
Older chats involving duplicate tool names may lose access to a previously loaded tool until the renamed handle is discovered; the fix is localized.

Look up streamed metadata by the original MCP tool name.
Collision handling gives the model a suffixed name. The tool-result builder preserves that name, and emitToolResults uses it as the metadata-cache key. The cache uses the original MCP name, so the lookup returns {}.
The raw result and structuredContent remain available. However, descriptor metadata such as ui.resourceUri is omitted from streamed output. In shared or minimal chat, this can prevent the MCP App widget from being identified and rendered without a tools/list prefetch.
Preserve the original MCP name through the tool-result path and use it for this lookup. This finding concerns streamed output only. It does not cover the separately inspected widget snapshot path.

Preserve the original MCP name on displaced variants.
When place moves a duplicate tool, it sets _mcpToolName but not _originalName. buildToolCatalog reads only _originalName, so the displaced tool receives serverId::tool__server instead of serverId::tool. An older chat with the legacy ID is then skipped during hydration, and load_mcp_tools returns it in notFound.
