PR 1 of the alerting tier 1 work (spec: docs/alerting-tier1-spec.md, see "PR 1 as built"). One rule can now hold a warning and a critical threshold, each with an optional recovery value and its own destinations, and open on "N of the last M checks". The editor UI comes in PR 2. Until then, tiers are set through the v2 API; the web app shows tiered rules, and saves them as long as the single-threshold fields are unchanged.

Behaviour

  • An incident opens at the tier the value breaches, then escalates or de-escalates as the same incident.
  • Who gets notified:
    • Trigger and escalation reach every tier at or below the new severity. A destination in both tiers is notified once.
    • De-escalation and resolve reach every tier up to the incident's peak, so the pager that heard the critical hears it calm down.
    • Reminders reach only the current tier.
    • Resolves from a rule edit or a vanished group follow the same rule.
  • Recovery value: the value must cross back over it before a tier clears. Between the trigger and the recovery value, the incident neither resolves nor reopens.
  • N of M: breaches_out_of counts the required breaches over the last M checks. null keeps today's "N in a row".
  • Rule edits:
    • An edit that changes the comparator or a tier value resets the group's tick history and counters in the rule-write transaction, so ticks judged against the old condition never count.
    • An incident whose tier was removed reads as the tier the rule kept. The move is announced on its first breaching tick.
  • Linked issue: opening at or escalating to critical raises a detector-set issue to critical. Manual and AI severities are left alone.
  • Existing rules behave identically. A rule without tiers reads as one tier from its legacy columns, and a property test replays random check sequences through the old consecutive-count machine and the new planner with identical results.

Design notes

  • No new event types. The iOS app decodes AlertEventType and AlertIncidentTransition as closed enums.
    • Escalation is delivered as trigger at the new severity, de-escalation as renotify.
    • Check rows record a severity change as continued; the new alert_checks.Level says which tier breached.
  • History replaces counters. Per-group state stores a short tick history (history_json). A steady group's history does not change, so the state upsert is still skipped (confirmed on the preview, see the verification comment).
  • Legacy columns stay in step. severity, threshold, threshold_upper and destination_ids_json are derived from the tiers on every write, so older readers and the "destination in use" check keep working. A later PR drops them.
  • v2 PATCH keeps tiers.
    • Single-threshold fields edit a one-tier rule's tier in place.
    • On a two-tier rule they return a 400 instead of collapsing it, unless they echo the derived values unchanged.
    • The MCP update_alert_rule tool follows the same rules.
  • Preview replays the scheduler's planner. Firing spans carry a severity and split where the severity changes.

Migrations

  • Postgres (20261015225936_alert_severity_tiers): four nullable columns, no backfill and no row rewrites.
    • The columns: alert_rules.thresholds_json, alert_rules.breaches_out_of, alert_rule_states.history_json, alert_incidents.peak_severity.
    • A null thresholds_json reads as one tier from the legacy columns. A backfill could go stale if an old worker edited a rule mid-deploy, so it waits for the drop-columns migration.
  • Warehouse: alert_checks.Level in the datasource, plus ClickHouse migration 0041 for existing self-hosted servers.
  • Local schema: bumped to v32 with an ADD COLUMN step.

Most of the line count is generated: the migration snapshot (~12.7k) and the local schema v32 snapshot (~2.1k). The hand-written change is about 2.7k added lines, about 45% of them tests, plus the spec.

Verification

  • Typecheck: clean for alerting-core, domain, backend, db, api, ai, web and cli.
  • Tests (Vitest, all passing):
    • alerting-core: 29
    • backend alerts and issue-hub: 394
    • db (incl. parity): 53
    • domain http and ClickHouse migrations: 318
    • api v2: 232
    • web alerts: 146
    • ai mcp: 753
    • ios:openapi:check and clickhouse:schema:check also pass.
    • Every bug fixed during review has a test that fails without the fix.
  • Migration on a real Postgres 17, seeded with pre-tier rules, states and an open incident:
    • It applied, and a re-run was a no-op.
    • All rows decoded through the table definitions.
    • Each rule read back as exactly one tier.
    • A legacy state row opened at N, as the counter machine did.
  • Preview (pr-1509): a separate agent tested the v2 API, the preview replay, the scheduler (a real warning, critical, warning cycle) and the web pages in light, dark and mobile. The results and traces are in the verification comment.

Open

  • Whether the pager integration updates the severity of an alert re-triggered with the same dedup key. This decides whether escalation needs a dedup key per severity.
  • The legacy single-threshold fields are not yet marked deprecated in the OpenAPI document (spec section 8).
  • Follow-ups that predate this PR:
    • v2 returns AlertValidationError as a bare "Invalid alert rule" without its details.
    • A rule's last_evaluated_at is only filled when the rule has an evaluation error.
  • Editor, template variables, change conditions, silences and incident actions come in later PRs (see the spec's rollout section).

Summary by CodeRabbit

  • New Features
    • Alert rules now support warning and critical threshold tiers, with separate recovery thresholds and notification destinations.
    • Configure alerts to trigger when a chosen number of checks breach within a rolling window, rather than only on consecutive breaches.
    • Incidents and alert previews now show severity levels, including the highest severity reached. Severity changes update notifications and associated issues.
    • PagerDuty notifications distinguish critical alerts and close critical notifications when an incident resolves or drops to warning.
  • Bug Fixes
    • Rule edits preserve tier settings and breach counts where appropriate, and prevent legacy single-threshold edits from collapsing multi-tier rules.

…on one rule

A rule now carries up to two severity tiers, each with its own value,
optional recovery value and destinations, and can open on N breaches out
of the last M checks. The incident opens at the tier the value breaches
and escalates or de-escalates in place; a critical incident also notifies
the warning tier's destinations, and resolve reaches everyone notified.

- alerting-core: tier classification (breach / hold / clear), a per-group
tick history replacing the two counters, and a lifecycle planner that
replays counter-only state rows exactly. A property test pins single-tier
rules to the old consecutive-count machine.
- Postgres: alert_rules.thresholds_json (backfilled) and breaches_out_of,
  alert_rule_states.history_json, alert_incidents.peak_severity. Legacy
single-threshold columns stay in step for pre-tier readers.
- Escalation is delivered as a trigger and de-escalation as a renotify,
and check rows record a severity change as "continued" with the new
alert_checks.Level column, so clients that decode those enums keep working.
- v2 API: thresholds and breaches_out_of on rules, severity on preview
spans, points and checks, peak_severity on incidents. PATCH keeps a
rule's tiers and rejects single-threshold edits on a two-tier rule.
- Preview replays the same planner, so firing spans carry their severity.
- Local schema v32 adds alert_checks.Level.

Spec for the remaining tier 1 work: docs/alerting-tier1-spec.md.

Tiered alerts can, in edge cases, send a PagerDuty event with the wrong key or miss a critical escalation page. Removing a destination while an incident is open can also leave a critical alert unresolved at that destination. Address these delivery issues, or explicitly accept them, before merging.

The v2 thresholds field stays a bounded plain array on the wire instead of
a non-empty tuple, so the generated spec carries no prefixItems for the
Swift client generator; servers narrow it after decode. The iOS fixture
rule gains its one tier.
- Reminders after a de-escalation reach only the current tier; the
de-escalation itself and resolves still reach every tier up to the peak.
- Resolves from a rule edit or a vanished group use the incident's tier,
so a warning incident no longer resolves to the critical pager.
- Preview firing spans split at each severity change, as the spec says.
- Drop the thresholds_json backfill: readers already derive one tier from
a NULL, and the backfill could go stale while old workers still edit rules.
- Remove the SEVERITY_RANK alias and the identity coreTiers mapper, and
revert formatter churn in the alerts lab fixture.

The tier planner, normalizer, delivery routing, preview replay and migrations read and consistent, but the v2 routes, the web editor reads and four review groups went unread.
quality 98/100 · 1 note · tests covered · risk high · 1/1 new units observable

Adds warning/critical tiers, recovery values and N-of-M to alert rules while keeping legacy columns, event types and the check counters in step. The tier planner, normalizer, delivery routing, preview replay and migrations read here are consistent; the v2 routes and the web changes went unread.

  • planAlertLifecycle emits escalated/deescalated and a per-tick severity from a persisted tick history
  • ruleAtSeverity narrows a rule to the incident's tier, reaching every destination up to the peak
  • simulateFiringSpans now replays planAlertLifecycle and splits spans at each severity change
  • alert_checks.Level and alert_incidents.peak_severity record the breaching tier

correctness · apps/web/src/lab/worst-case-docs.ts:160

rule(n, name, threshold, tags) still sets threshold from its parameter and severity: "critical", but the added tier pins value: 0, so every fixture rule decodes with thresholds[0].value !== threshold — a state alertThresholdLegacyFields (packages/domain/src/http/alerts.ts:253) never produces, since it derives threshold from the tiers. - MCP update_alert_rule carries tiers and breachesOutOf forward instead
of rebuilding one tier, and refuses single-threshold edits that would
merge a two-tier rule. New destinations join the warning tier.
- An incident whose severity the rule no longer has reads as the kept
tier, so a severity edit no longer sends a false resolve.
- Editing the comparator or a tier value clears the tick history, so
ticks judged against the old condition do not count toward N of M.
- Escalation raises the linked issue's severity while it is detector-set.
- Preview spans after a resolve no longer date back into the last span.
- Tiers are strictly ordered for gte/lte, and a critical recovery value
may not sit past the warning threshold.
- An explicit threshold_upper on PATCH is validated, not dropped.
- ClickHouse migration 0041 adds alert_checks.Level on existing servers.
- The v2 tier list decodes as non-empty; shared tier mappers live in
domain, and pre-tier rows decode through the typed helper.

A two-tier rule that opens straight at critical gave its issue the
critical severity but the warning threshold. The issue upsert now reads
severity and threshold from the same tier view as the incident row.

The tier machine replaces every rule's lifecycle, but a property test pins legacy equivalence and the tier, PATCH and HTTP paths are tested.
quality 98/100 · 1 note · tests covered · risk medium · 1/1 new units observable

One rule can now carry a warning and a critical tier, each with an optional recovery value and its own destinations, and open on N of the last M checks; the lifecycle machine replans from a tick history. Safe to merge; the only open item is the info-level lab fixture nit.

  • planAlertLifecycle replans opens, escalation and de-escalation from a per-group tick history
  • Legacy severity/threshold/destinationIds and the v2 single-threshold shape derive from the tiers
  • v2 PATCH carries tiers forward and 400s a collapsing single-threshold edit
  • Postgres adds thresholds_json, breaches_out_of, history_json, peak_severity; ClickHouse adds alert_checks.Level

…ier moves

- A condition edit resets history and counters inside the rule-write
transaction, so warning-only ticks no longer replay as critical and a
tick that claimed the old config cannot write stale history back.
- Resolves map the incident's severity onto the tier the rule kept, so
removing that tier no longer drops the resolve. A move is announced as
escalation or de-escalation on its first breaching tick.
- Opening at critical and escalating to critical raise a detector-set
issue the same way, bump updatedAt and leave wontfix issues alone.
- MCP edits drop a recovery value the new threshold or comparator makes
invalid, since MCP cannot set one yet.

- The state reset is decided inside the rule-write transaction against
the row as stored, so a concurrent edit built from a stale read cannot
restore the old condition without resetting the ticks.
- A critical incident that reopens a wontfix alert issue raises its
detector severity after the reopen, not before it.
- MCP edits on a two-tier rule drop recovery values a comparator change
invalidates, as one-tier edits already do.

The tiered planner, legacy-column mirroring and the new suite match the spec; I did not read a line-by-line diff of every file outside the since-list.
quality 98/100 · 1 note · tests covered · risk high · 2/2 new units observable

Adds warning/critical threshold tiers, recovery values, per-tier destinations and N-of-M breach windows to alert rules, escalating and de-escalating one incident while keeping the legacy columns in step. The new code is consistent with its spec and no new defect surfaced; the open lab-fixture finding is still unfixed at this head.

  • planAlertLifecycle folds ticks into per-tier breach/recovery state instead of two counters
  • An incident escalates and de-escalates in place, notifying the tiers at or below it
  • thresholds_json, breaches_out_of, history_json, peak_severity added nullable, unbackfilled
  • v2 PATCH edits a one-tier rule in place and 400s on a two-tier rule

…PATCH

A client that PATCHes back the severity, threshold, threshold_upper or
destination_ids it just read got a 400 on a two-tier rule. Fields equal
to the derived values are not edits, so a rename from the web form or a
read-modify-write client keeps working.

Verification on the preview (pr-1509)

A separate agent tested the preview through the v2 API and the web app, deployed from 4d59579. Since then the only pushed changes are a lab fixture and the PATCH echo fix below, which has its own route test.

Flow Result
Create a two-tier rule (warning 0.02 with recovery 0.01, critical 0.05, 2 of 4) Pass. Tiers come back lowest first; severity, threshold and destination_ids are derived.
Validation 400s: critical not worse (incl. equal gte), recovery on the wrong side, critical recovery past warning, two tiers on between/eq, breaches_out_of out of range, duplicate severity, three tiers Pass
PATCH: enabled keeps tiers; single-threshold edits on a two-tier rule return 400; thresholds replaces; a legacy rule edits its one tier in place and keeps its recovery value; explicit threshold_upper on gt returns 400 Pass
Preview: level per point, spans split at severity changes (warning, then critical, then warning) Pass
Scheduler: opened at warning, escalated to critical (trigger to both tiers), de-escalated (renotify to the pager), peak_severity: critical, checks carry level Pass
Web: rules list, tiered and legacy rule pages, inbox, incident page in light, dark and 390px Pass, no page errors and no 4xx/5xx from the API
Saving a two-tier rule from the web form A readable 400 toast. The PATCH echo fix (1457778) makes an unchanged single-threshold field a no-op, so a rename from the form now saves.

Before that, the migration was applied to a real Postgres 17 seeded with pre-tier rules, states and an open incident. It applied and re-ran as a no-op, every row decoded, each rule read back as one tier, and a legacy state row opened at N exactly as the counter machine did.

Traces (Maple, deployment.environment = pr-1509)

  • Scheduler tick: AlertsService.runSchedulerTick took 817 ms for 5 rules. AlertsService.evaluateRule p95 is 343 ms, almost all warehouse time. AlertsService.processEvaluation p95 is 237 ms, typically 0 to 112 ms.
  • Electric write gate: about 70 rule evaluations, but only 18 UPDATE alert_rule_states, all from groups whose values were moving. Steady groups do not write.
  • v2 alert routes (root spans): GET p95 350 ms, POST 497 ms, PATCH 989 ms (read, merge, then a locked write), DELETE 644 ms.
  • Errors: only AlertDeliveryRejectedError from the test webhooks on example.com (405), which is expected.
  • The slowest alerting.lane traces (up to 9.8 s) are ServiceMapRollupService.runRollupTick, which this PR does not change.

The 11-line echo guard is a public PATCH-semantics change, pinned by a route test that fails without it; only the no-op wording for a tiered rule is a judgment call.
quality 100/100 · no findings · tests covered · risk medium

Narrows the v2 alert-rule PATCH so single-threshold fields that merely echo a tiered rule's derived values are a no-op instead of a 400, and regenerates the route test around it. The two files changed since the last review are correct as written.

  • mergeThresholds ignores echoed severity, threshold, threshold_upper and destination_ids on a tiered rule
  • A real single-threshold edit on a two-tier rule still returns 400
  • Route test pins both the echo no-op and the explicit-bound 400

Assert the echoed derived fields.

The echoed PATCH test does not detect regressions that change severity, threshold, threshold_upper, breaches_out_of, or destination_ids in the response. Add targeted assertions for the derived fields and breaches_out_of. Whole-body equality is broader than required.

The fixture has one destination, so reversing destination_ids does not exercise order-insensitivity. A changed-destination case is not required for this test's intended no-op behavior because the PATCH contract already compares destination membership without considering order. Related tests should cover actual destination edits if that behavior requires protection.

Preserve a PagerDuty deduplication key for each escalation severity.

The escalation path sends a trigger with the incident's existing dedupeKey. PagerDuty correlates that event with the existing alert. It does not guarantee a new notification, and acknowledged alerts do not generate additional notifications for new trigger events. This can suppress the required critical escalation notification.

Use a severity-specific PagerDuty key for escalation, and retain every key used during the incident so resolve events close both the warning and critical alerts. Changing only this field would leave the earlier PagerDuty alert unresolved.

Re-triggering the warning alert's dedup key only updates that alert, and an
acknowledged alert does not page again, so an escalation could go unheard.
A rule with both tiers now pages critical under <dedupe>:critical and
closes it through a follow-up resolve when the incident falls back to
warning or resolves. Single-tier rules keep the one key their open alerts
were raised under. HTTP transports gain idempotent follow-up requests.

One delivery edge case (a rule dropping its critical tier) leaves the PagerDuty alert open, and the runner's new follow-up loop has no test.

PagerDuty now pages a tiered rule's critical under its own dedup key and sends follow-up resolves so that key closes on de-escalation or resolve. The wiring is sound, but a rule that loses its critical tier mid-incident leaves the critical alert open.

  • pagerDutyDedupKey pages a two-tier rule's critical under <dedupeKey>:critical

Adds warning/critical tiers, recovery values and N-of-M to one alert rule: a pure tier planner in @maple/alerting-core, tier-aware incident open/escalate/de-escalate/resolve in the scheduler, plus nullable Postgres and ClickHouse columns and the v2 contract. 🗄️ Data Integrity & Integration | 🟠 Major | 🏗️ Heavy lift

Keep destinations that received a critical trigger until its alert closes.

If a rule edit removes a critical-only destination while an incident is open, ruleForResolve(normalized, openIncident) selects destinations from the new rule. The resolve never reaches that destination, even when peakSeverity is critical. The severity-change path at Line 2721 has the same limitation. Preserve the incident’s notified destinations, or close their alerts when the edit removes them.

Commit an escalation and its delivery rows together.

On a critical escalation, the incident update completes before queueIncidentNotifications prepares and inserts the trigger delivery. If preparation or insertion fails, the incident remains critical without a queued critical trigger. The next evaluation sees a continued incident rather than a new escalation. Prepare the deliveries before the update and commit the severity change with its delivery rows in one transaction.

The PR preview database had already applied 20261015225936_alert_severity_tiers,
so the regenerated name read as rewritten history and failed the preview
deploy. The original name still sorts after main's GCP migrations; it keeps
the regenerated snapshot so the folder has a single leaf.
- Deliveries snapshot the rule's tiers and the incident's peak severity in
the queued payload, and dispatch reads the event's own severity, so a
late or retried critical trigger still pages the critical PagerDuty key.
- A resolve also reaches every destination the incident already notified,
so a rule edit that drops a destination cannot leave its alert open.

🟡 Confidence 6/10 · needs attention
The new dispatch-time severity choice closes the critical PagerDuty key on a delayed warning delivery; no test covers a dispatch after the incident moved on.
quality 88/100 · 1 warning · 1 note · tests partial · risk medium

Adds warning/critical tiers, recovery values and N-of-M to one rule, and keys each queued notification off the event's own tier. Not safe as-is: a delayed warning-tier delivery now closes the critical PagerDuty alert, and the earlier countersFromHistory finding still stands.

  • prepareIncidentDeliveries adds every destination a resolve already notified
  • Dispatch keys a delivery off the queued event's severity and rule tiers (dispatchTierFields)
  • StoredDeliveryPayloadSchema snapshots rule.severities and peakSeverity
  • Migration adds four nullable columns with no backfill

Findings

🟠 Warning · F4 · A warning event dispatched after an escalation closes the critical PagerDuty alert

correctness · packages/backend/src/services/alerts/AlertsService.ts:2017-2019

severity is now taken from the queued event, so a warning-tier trigger or reminder that the drain (or a retry) dispatches after the incident escalated reaches pagerduty.ts:104 as warning while incidentPeakSeverity is critical, and followUps sends resolve for the :critical dedup key — the critical page is closed while the incident is still critical (criticalHasOwnKey is already true for a two-tier rule, so the peak only widens it). Before this swap the incident's severity won here, so the same delayed delivery could not close the critical key. The mirror case also leaks through: a queued critical-tier event retried after a de-escalation re-triggers the critical key the de-escalation just resolved.

Decide the sibling-key follow-up against the incident's current tier, not the event's: carry the incident's severity in the dispatch context (it is already read as `incidentRow` here) and have `pagerduty.ts` close/open the `:critical` key only from that value, leaving the primary `dedup_key` on the queued event's severity.

…rrent tier

A warning event delivered after an escalation closed the critical page,
and a critical event retried after a de-escalation re-opened it. Dispatch
now carries the open incident's severity: the critical alert closes only
when the incident is at warning, and the primary key never reaches above
the incident's current tier.

Audit counters no longer count a recovery-band check as healthy.

🟢 Confidence 8/10 · likely safe to merge
The delta is two small, well-reasoned fixes in the tier planner and the PagerDuty key choice, each pinned by a test; I read both and their callers.
quality 100/100 · no findings · tests covered · risk medium · 1 file not read

Fixes the two open findings: the audit counters no longer treat a recovery-band tick as healthy, and PagerDuty picks and closes its critical key from the incident's current tier rather than the queued event's. Safe to merge.

  • countersFromHistory counts only ticks where every tier is clear as healthy (tiers.ts:139)
  • PagerDuty's dedup key and critical-key close now use the incident's current tier (pagerduty.ts:31, pagerduty.ts:112)

Before merge

Fixed since the last review

  • ✅ F3 · countersFromHistory counts a recovery-band tick as healthy
  • ✅ F4 · A warning event dispatched after an escalation closes the critical PagerDuty alert

What was checked
  • incidentSeverity is set only for an open incident row; AlertIncidentStatus is open/resolved with holds inside open (alerts.ts:105), so held incidents still suppress the critical close
  • keySeverity demotes only critical→warning, so a critical event retried after a de-escalation pages the base key and cannot re-open the closed critical alert (pagerduty.ts:31)
  • closesCritical excludes test events (pagerduty.ts:112), so a send-test on a two-tier rule still fires only the base-key test trigger
