[None][fix] serve: fail a request whose generation degenerates into a token run - #21062
A model can fall into repeating a token it never writes in valid output, until its token budget runs out:
Kimi-K3 has been seen to write its pad token [PAD] (id 163839, its last vocabulary id) thousands of times in a row, starting mid-sentence.
For DeepSeek-V4.1-Flash the same failure would repeat <|▁pad▁|> (id 2) or its last vocabulary id, <|place_holder_mm_span_0442|> (id 129279), a multimodal placeholder that no prompt uses.
These are special tokens, so they vanish from the detokenized text. The client receives an empty or truncated reply as a normal answer. An agent client keeps it in the conversation and sends it back on every later turn. The Codex CLI, for example, keeps every output item once its response.output_item.done arrives, whatever the terminal event says.
This PR makes such a run fail the request without output. Models declare their degenerate tokens; models that declare nothing are unchanged.
Declaration. A new serving-extension hook, ServingExtension.degenerate_tokens(hf_config), returns DegenerateTokens(token_ids, run_length=20) or None (the default). Two models declare tokens:
deepseek_v41 (new extension): the config's pad_token_id and the last vocabulary id, (2, 129279) for the release checkpoint.
kimi_k3: the last vocabulary id, [PAD].
The declaration lives in the extension registry because that is where per-model serving behavior is keyed on model_type. The checkpoints do not mark any token as degenerate, and the ids are not reliable elsewhere:
The V4.1 tokenizer reports <|end▁of▁sentence|> as its pad token, while the config says 2.
KimiK3Config defaults pad_token_id to 0 when the checkpoint leaves it out, so the K3 id is taken from the vocabulary size.
The V4.1 renderer is a custom tokenizer selected by a flag, so a declaration on it would be lost without that flag.
Stop. For each declared id, the server adds a stop word of run_length copies of that id to every generation request (SamplingParams._set_degenerate_token_runs), after the request's own stop words. The engine matches these server-added rules only against generated tokens, in streaming and non-streaming requests. A prompt suffix never contributes to the run; ordinary request stop words retain their prompt-spanning behavior. The original generation boundary survives native pause/recompute, so a run interrupted after ten generated tokens stops after ten more. The rule marker is preserved through executor conversion, rank broadcast, and child-request cloning, and both host and CUDA matching enforce the boundary. Matching is by token id: text that spells such a token (for example [PAD] written in code) is ordinary tokens and never matches. A run of 19 does not stop the generation.
Failure. When a generation stops on such a run, the result layer raises a RequestError instead of reporting a stop. A run completing exactly at max_tokens or max_seq_len also fails, even though both samplers report LENGTH at that boundary. The result layer preserves earlier matching user stop rules and the ignore_eos opt-out. The result keeps the error, so every later wait on it raises it too, and none of the output is returned. If other sequences of the request are still running, they are aborted. The message reaches the server as text, prefixed when a postprocessing worker built the response, so the server recognizes it by text, the same way as context_length_exceeded.
What clients see.
Non-streamed requests, on every route: HTTP 500 InternalServerError (create_error_response). Anthropic Messages: a 500 api_error.
Chat Completions streams, including the Harmony chat route: an in-stream {"error": ...} event and [DONE], instead of a dropped connection. The OpenAI Python client raises it as an APIError.
Anthropic Messages streams: an error event (api_error) with the message. The stream reframer now recognizes an upstream error frame; before, it reported it as a malformed chunk.
Responses streams: error and response.failed, with no output in the failed response. The step that hit the run emits nothing, so the items still open are never closed. Items closed earlier in the stream are unchanged.
Notes.
A run is of one id. A run that mixes several declared ids is not matched.
ignore_eos drops all stop words of a request, so it drops the runs too.
For a declared model, every request carries multi-token stop words. They use the sampler's stop-word history and skip its in-graph greedy sampling and its batched host update.
DeepSeek-V4.1 now has its own extension class, which is part of the rendering fingerprint. A router and workers on different versions report different fingerprints for V4.1.
The completions stream error event is now built by the same helper as the chat one; its content is unchanged.
Test Coverage
Direct-submission prompt metadata (55140dade3): 125 focused CPU tests pass across degenerate-token runs, stop-output trimming, serving errors, and worker behavior. Real worker/proxy submissions reproduced a false error for 19 prompt pad tokens plus one generated pad when the postprocessing count was unset. Shared result initialization now fills that missing count before dispatch; explicit counts (including zero), generate_async, and genuine 20-token generated runs are covered. Formatting/lint, full mypy (16 sources), and normal commit/DCO hooks pass.
Prompt-inclusive length-boundary regression (d675add309): 79 focused CPU tests pass across degenerate-token runs, stop-word side-output trimming, and serving errors. Two cases failed before the repair: a returned prompt with 19 pad tokens plus one generated pad at max_tokens=1, in direct results and postprocessing workers. Generated-only output, incremental streaming, and genuine 20-token generated runs remain covered. The result check excludes prompt tokens from the run count. Full mypy (16 source files) and normal commit/DCO hooks pass.
tests/unittest/executor/test_degenerate_token_runs.py (new, CPU, added to l0_cpu.yml):
The runs are stop words of token ids, after the request's own stop words, also when stop strings are tokenized.
The engine's host-side stop rule (meet_stop_token_criteria) stops on the last copy of a run, and not on a shorter run or on text that spells the token.
The result layer fails a generation that stopped on a run, keeps the error, aborts the request's other sequences, and lets the request's own stop token win.
The message builder and its detector, also on a prefixed message.
tests/unittest/llmapi/apps/test_degenerate_generation_errors.py (new, CPU, added to l0_cpu.yml). It drives the real route handlers with an engine stub that fails the way the result layer does, in the server process and through a postprocessing worker:
The runs reach the engine on the chat, Harmony chat, completions and Responses routes. A model that declares nothing keeps its stop words.
Non-streamed requests fail with HTTP 500 on every route; the OpenAI Python client raises InternalServerError.
Chat streams (both chat routes) end with an error event and [DONE]; the OpenAI Python client raises.
Anthropic streams end with an error event and no message_stop; non-streamed requests get a 500 api_error.
Responses streams end with error and response.failed with no output, and the item opened before the run is never closed. The route re-raises the failure after those events, so this test reads the route's body directly.
tests/unittest/llmapi/apps/test_serving_extensions.py: the V4.1 declaration resolved from a config.json through load_pretrained_config, the V4.1 and K3 ids from other config shapes, models that declare nothing, and the validation of a declaration.
On a build of current main, the three files above plus test_anthropic_adapter.py, test_responses_streaming_events.py and tests/unittest/api_stability/test_llm_api.py pass: 351 tests, against 304 for the four existing files on main.
Pause/recompute and length-boundary regressions (2922cfffcf): 218 native tests pass on H100 across the executor, request propagation, pause lifecycle, and CUDA finish-reason suites; 39 related result-trimming and serving-error CPU tests pass. Five focused defect cases failed before this repair, including CUDA sampling with one and three candidate tokens. The tests cover the production 20-token threshold, native pause after ten tokens, both length limits, user-stop precedence, and ignore_eos. Full mypy checking of 16 source files and normal commit/DCO hooks pass.
Generated-token boundary regression (9120d1f139): all 206 focused tests pass on H100 across test_degenerate_token_runs.py, test_request_utils.py, and test_finish_reasons_fused.py. The tests exercise the production 20-token threshold with one-token and three-token speculative batches, partially accepted draft batches, mixed ordinary/generated-only rules, rank broadcast, child cloning, and ignore_eos. Removing the device boundary guard makes both new CUDA cases fail. Normal commit/DCO hooks and full mypy checking of 16 source files pass.
Current-main validation (3a3a200e67, merged main 5d86481f60): kept both Anthropic error/ping helpers, special-token decoding before degenerate-token stopping, both DeepSeek-V4.1 namespace and degenerate-token behaviors in one extension, and both test groups. With compiled bindings, 289 focused executor/route/extension/adapter tests and all 9 parser-decoding tests pass. Twenty additional grammar cases fail with the same unsupported qwen_3_coder format on unchanged main; there are no new failures in the comparison. Focused and normal commit hooks pass. CPU/single-GPU CI is being requested; full coverage still requires the approval label.

ServingExtension.degenerate_tokens(hf_config) lets model extensions declare token IDs and a run length. DeepSeek V4.1 declares its pad token and final vocabulary ID; Kimi K3 declares its final vocabulary ID. The default hook returns no declaration.
The server adds generated-token-only stop rules after request stop words. A matching run raises and records a RequestError; ignore_eos disables these rules. The Torch sampler carries the generated-only rule count through request conversion, broadcasting, and recomputation so prompt tokens do not trigger a match.
Degenerate-generation errors are handled across OpenAI-compatible, Harmony, Anthropic Messages, and Responses routes. Streaming routes emit error events; non-streaming routes return errors.
The supplied evidence includes reported test runs, but also reports failed, unstable, and aborted CI pipelines. Failure details and current review-finding counts are unavailable.
QA Engineer Review
Added tests/unittest/executor/test_degenerate_token_runs.py and tests/unittest/llmapi/apps/test_degenerate_generation_errors.py. Modified tests/unittest/llmapi/apps/test_serving_extensions.py, tests/unittest/_torch/pyexecutor/test_request_utils.py, and tests/unittest/_torch/sampler/test_finish_reasons_fused.py.
Added the executor and LLM API error tests to tests/integration/test_lists/test-db/l0_cpu.yml. The serving-extension tests are also listed there. The fused sampler test is listed in tests/integration/test_lists/test-db/l0_a10.yml. No manual-QA list entry was found.
Coverage includes stop-rule construction and matching, prompt exclusion, length boundaries, request abort and error handling, route responses, model declarations, request propagation, and sampler recomputation. Reported validation includes focused H100 tests for generated-token boundary behavior. CI failures and aborted runs remain unexplained.
Coverage verdict: needs follow-up. Resolve the reported CI failures and confirm coverage for the full default run length across relevant execution paths.

… token run
A model can fall into repeating a token it never writes in valid output
(its pad token, an unused placeholder) until its token budget runs out.
The tokens are special, so the client receives an empty or truncated
reply as a normal answer and keeps it in the conversation.
A serving extension now declares such tokens
(ServingExtension.degenerate_tokens): DeepSeek-V4.1 its pad token and its
last vocabulary id, Kimi-K3 its [PAD]. The server adds a stop word of 20
copies of each id to every generation request, matched by token id after
the request's own stop words. A generation that stops on one fails with a
RequestError instead of reporting a stop, so none of its output is
returned:
- non-streamed requests get HTTP 500;
- chat streams, also on the Harmony route, end with an error event and
[DONE];
- Anthropic Messages streams end with an error event;
- Responses streams end with error and response.failed, without output.
Models that declare nothing are unchanged.

Walkthrough
The change adds model-declared repeated-token stop conditions. The sampler treats server-added conditions as generated-only. When a configured run triggers, the executor raises a request error. Serving routes map the error to HTTP or streaming responses, and the Anthropic adapter extracts messages from upstream OpenAI error frames.

Sampling parameters add repeated-token stop words and typed stop reasons. The result layer turns a matching degenerate-token reason into a terminal RequestError, including at a length boundary when EOS is not ignored. Tests cover stop matching, precedence, and error state.
Requests carry the count of generated-only stop words through conversion, child creation, and broadcast. Stop matching uses prompt and sequence lengths to prevent those rules from matching prompt tokens. Tests cover request propagation and fused matching.
Routes apply extension-declared run limits and handle matching errors in HTTP and streaming responses. The Anthropic adapter extracts messages from upstream error frames. Route tests and the CPU test list cover these paths.

…the Kimi-K3 x Claude Code full tier
- F01: R-04 was noted with M-02 whenever a degenerate run showed in the
delivered text or in the joined raw record, so a reply the server
correctly failed on a [PAD] run (NVIDIA#21062: an error event, nothing
delivered, the run only in the partial raw record the hook writes when
the request ends) turned runtime FAIL. X-09, documented as that case,
was implemented as a run delivered in the text. R-04 now fails a run
only when it reached the client: in what was delivered, or in the
generation of a reply delivered as an answer (completed, or cut at its
token budget). A run only the raw record shows, in a reply that ended
without an answer (an error, a failure, no terminal event), is the
model's (M-02) and is counted in the R-04 record as stopped, with
locators. The raw loader reads how a record says its generation ended
(partial with its end_reason, or aborted): a reply joined to such a
record is held to RAW-05, a prefix of what was written, whatever its
terminal says (C-13 stopped_raw).
- F02: an empty or missing --build-id read as undeclared, so the
revision check (I-05) and its inventory blocker were skipped silently,
and verdict.json showed neither the declared id nor the evidence.
Replies that carry a build_id while no revision is declared (not
given, empty or blank) now make inventory BLOCKED with a reason naming
--build-id. verdict.json records a revision block (--build-id as given,
whether it was checked, the build ids the replies carry) and the
evidence read: every shard with its role, size and SHA-256, a digest
over the sorted "<role> <sha256>" lines that does not depend on where
the shards lie, and the SHA-256 of a MANIFEST.sha256 beside the
evidence (recorded, not verified). Claims for correct inputs do not
change. smoke.md's fast verdict now passes --build-id.
- F04 (docs): with several generation workers, USG-06 compares only the
replies whose engine id named one raw record. validate.md states the
limit, with the full tier's numbers, next to the 2026-10-28 join.

The CPU test covers only the host-side predicate and uses a shortened run. Existing CUDA tests cover generic stop-word matching, but they do not pass a production SamplingParams degenerate run through the sampler at the 20-token threshold or cover the speculative-decoding path. Add non-speculative and speculative cases that assert FinishReason.STOP_WORDS at 20 repeated tokens. The existing CPU result-layer test already covers conversion of that reason to RequestError; a full model-generation test is not required. Place the test in the already-selected tests/unittest/_torch/sampler suite.

Preserve the degenerate stop reason at the length boundary.
If a 20-token run completes on token 20 of max_tokens=20, the stop matcher sets STOP_WORDS, but the length check overwrites it with LENGTH. GenerationResultBase then returns the degenerate output instead of raising RequestError. The host path in tensorrt_llm/_torch/pyexecutor/sampler/sampler_features.py also checks length before stop words. Give a matching degenerate run precedence in both paths, and test a run that completes on the last allowed token.

[PAD](id 163839, its last vocabulary id) thousands of times in a row, starting mid-sentence.<|▁pad▁|>(id 2) or its last vocabulary id,<|place_holder_mm_span_0442|>(id 129279), a multimodal placeholder that no prompt uses. These are special tokens, so they vanish from the detokenized text. The client receives an empty or truncated reply as a normal answer. An agent client keeps it in the conversation and sends it back on every later turn. The Codex CLI, for example, keeps every output item once its
response.output_item.donearrives, whatever the terminal event says. This PR makes such a run fail the request without output. Models declare their degenerate tokens; models that declare nothing are unchanged.
Declaration. A new serving-extension hook,
ServingExtension.degenerate_tokens(hf_config), returnsDegenerateTokens(token_ids, run_length=20)orNone(the default). Two models declare tokens:deepseek_v41(new extension): the config'spad_token_idand the last vocabulary id,(2, 129279)for the release checkpoint.kimi_k3: the last vocabulary id,[PAD].The declaration lives in the extension registry because that is where per-model serving behavior is keyed on
model_type. The checkpoints do not mark any token as degenerate, and the ids are not reliable elsewhere:<|end▁of▁sentence|>as its pad token, while the config says 2.KimiK3Configdefaultspad_token_idto 0 when the checkpoint leaves it out, so the K3 id is taken from the vocabulary size.Stop. For each declared id, the server adds a stop word of
run_lengthcopies of that id to every generation request (SamplingParams._set_degenerate_token_runs), after the request's own stop words. Matching is by token id: text that spells such a token (for example[PAD]written in code) is ordinary tokens and never matches. A run of 19 does not stop the generation.Failure. When a generation stops on such a run, the result layer raises a
RequestErrorinstead of reporting a stop. A run completing exactly atmax_tokensormax_seq_lenalso fails, even though both samplers reportLENGTHat that boundary. The result layer preserves earlier matching user stop rules and theignore_eosopt-out. The result keeps the error, so every later wait on it raises it too, and none of the output is returned. If other sequences of the request are still running, they are aborted. The message reaches the server as text, prefixed when a postprocessing worker built the response, so the server recognizes it by text, the same way ascontext_length_exceeded.What clients see.
InternalServerError(create_error_response). Anthropic Messages: a 500api_error.{"error": ...}event and[DONE], instead of a dropped connection. The OpenAI Python client raises it as anAPIError.errorevent (api_error) with the message. The stream reframer now recognizes an upstream error frame; before, it reported it as a malformed chunk.errorandresponse.failed, with no output in the failed response. The step that hit the run emits nothing, so the items still open are never closed. Items closed earlier in the stream are unchanged.Notes.
ignore_eosdrops all stop words of a request, so it drops the runs too.Test Coverage
Direct-submission prompt metadata (
55140dade3): 125 focused CPU tests pass across degenerate-token runs, stop-output trimming, serving errors, and worker behavior. Real worker/proxy submissions reproduced a false error for 19 prompt pad tokens plus one generated pad when the postprocessing count was unset. Shared result initialization now fills that missing count before dispatch; explicit counts (including zero),generate_async, and genuine 20-token generated runs are covered. Formatting/lint, full mypy (16 sources), and normal commit/DCO hooks pass.Prompt-inclusive length-boundary regression (
d675add309): 79 focused CPU tests pass across degenerate-token runs, stop-word side-output trimming, and serving errors. Two cases failed before the repair: a returned prompt with 19 pad tokens plus one generated pad atmax_tokens=1, in direct results and postprocessing workers. Generated-only output, incremental streaming, and genuine 20-token generated runs remain covered. The result check excludes prompt tokens from the run count. Full mypy (16 source files) and normal commit/DCO hooks pass.tests/unittest/executor/test_degenerate_token_runs.py(new, CPU, added tol0_cpu.yml):meet_stop_token_criteria) stops on the last copy of a run, and not on a shorter run or on text that spells the token.tests/unittest/llmapi/apps/test_degenerate_generation_errors.py(new, CPU, added tol0_cpu.yml). It drives the real route handlers with an engine stub that fails the way the result layer does, in the server process and through a postprocessing worker:InternalServerError.[DONE]; the OpenAI Python client raises.errorevent and nomessage_stop; non-streamed requests get a 500api_error.errorandresponse.failedwith no output, and the item opened before the run is never closed. The route re-raises the failure after those events, so this test reads the route's body directly.tests/unittest/llmapi/apps/test_serving_extensions.py: the V4.1 declaration resolved from aconfig.jsonthroughload_pretrained_config, the V4.1 and K3 ids from other config shapes, models that declare nothing, and the validation of a declaration. On a build of current main, the three files above plus
test_anthropic_adapter.py,test_responses_streaming_events.pyandtests/unittest/api_stability/test_llm_api.pypass: 351 tests, against 304 for the four existing files on main.Pause/recompute and length-boundary regressions (
2922cfffcf): 218 native tests pass on H100 across the executor, request propagation, pause lifecycle, and CUDA finish-reason suites; 39 related result-trimming and serving-error CPU tests pass. Five focused defect cases failed before this repair, including CUDA sampling with one and three candidate tokens. The tests cover the production 20-token threshold, native pause after ten tokens, both length limits, user-stop precedence, andignore_eos. Full mypy checking of 16 source files and normal commit/DCO hooks pass.Generated-token boundary regression (
9120d1f139): all 206 focused tests pass on H100 acrosstest_degenerate_token_runs.py,test_request_utils.py, andtest_finish_reasons_fused.py. The tests exercise the production 20-token threshold with one-token and three-token speculative batches, partially accepted draft batches, mixed ordinary/generated-only rules, rank broadcast, child cloning, andignore_eos. Removing the device boundary guard makes both new CUDA cases fail. Normal commit/DCO hooks and full mypy checking of 16 source files pass.Current-main validation (
3a3a200e67, merged main5d86481f60): kept both Anthropic error/ping helpers, special-token decoding before degenerate-token stopping, both DeepSeek-V4.1 namespace and degenerate-token behaviors in one extension, and both test groups. With compiled bindings, 289 focused executor/route/extension/adapter tests and all 9 parser-decoding tests pass. Twenty additional grammar cases fail with the same unsupportedqwen_3_coderformat on unchanged main; there are no new failures in the comparison. Focused and normal commit hooks pass. CPU/single-GPU CI is being requested; full coverage still requires the approval label.PR Checklist

ServingExtension.degenerate_tokens(hf_config)lets model extensions declare token IDs and a run length. DeepSeek V4.1 declares its pad token and final vocabulary ID; Kimi K3 declares its final vocabulary ID. The default hook returns no declaration.RequestError;ignore_eosdisables these rules. The Torch sampler carries the generated-only rule count through request conversion, broadcasting, and recomputation so prompt tokens do not trigger a match.QA Engineer Review
