fix(adapters): capture codex usage when the hook path is unusable; count subagent rollouts (#775) - #895
Conversation
) The relay read stdin as text, which on Windows decodes with the locale code page: a UTF-8 transcript path either failed to decode (the whole payload became {}) or came out garbled. Both twins now read the bytes through a shared _read_payload and tag an unreadable payload with payload_error, which HookEvent carries for the wait loop to crumb.
…th is unusable (#775) A codex rollout's filename carries the hook session_id, so a missing or unusable hook transcript_path is resolved under $CODEX_HOME/sessions. The resolution sticks against a later unusable hook path, also runs at window death, and is crumbed (transcript-resolved / transcript-unresolved), as is an unreadable hook payload (hook-payload-unreadable).
Codex writes each subagent to its own rollout and leaves its usage out of the parent's total_token_usage, so the codex tally under-counted any session that spawned subagents. tally_codex_rollout now adds each child's last cumulative totals: children are rollouts in the parent's day dir and later ones (midnight crossings) whose first-line session_meta names the parent as session_id, or whose parent_thread_id chains into the tree. First-line facts are cached per path (bounded, oldest-first), so the budget guard's per-heartbeat tally re-reads only file bodies.

The codex tally skipped an unreadable subagent rollout, a failed sibling scan, or an unreadable parent first line with no record, so an under-counted session looked like a measured one. TokenUsage now carries the skipped sources (file name and exception class only, excluded from equality, add and to_dict), and the engine journals them on session-end as usage_unread. diagnostics drops the field to a presence flag because a rollout name embeds the session id.

Preserve failures while parsing Codex session metadata
When a rollout's first line contains invalid UTF-8 or malformed JSON, json.loads() raises UnicodeDecodeError/JSONDecodeError, but this handler converts the failure to the same None used for an unrelated file. If the affected rollout is the parent or a child, its descendants or usage are omitted without populating TokenUsage.unread, so session-end presents the lower total without the intended degradation marker. Propagate or otherwise distinguish parse failures so the tally can record the unread source.

Fixed in 2546e9a, together with the matching CodeRabbit thread. A complete first line that fails to parse now lands in unread for both the parent (whose children then go uncounted) and siblings. A child's body errors were already caught as (OSError, ValueError).

Surface directory scan failures during sibling discovery
Path.glob() suppresses OSError raised while scanning a directory, so the caller's surrounding except OSError cannot add usage_unread. For example, a known parent rollout can remain openable in an executable-but-unreadable day directory while sibling enumeration silently yields nothing; all subagent usage is then omitted and the total looks complete. Use an enumeration path that propagates scan failures or explicitly records them.

Fixed in 2546e9a. Sibling day dirs are now listed with iterdir() plus an fnmatchcase filter, not glob(). A missing day dir is skipped. Any other OSError names that dir in unread by its date path (2026/10/02/), and the remaining dirs are still searched. This also fixes the old except OSError around the scan, which was dead code because of the glob behaviour you describe. Test: test_codex_tally_names_unlistable_day_dir, ablated.

Resolve rollouts on every window-death return
This resolution call covers only the ordinary no-event death check. If the initial liveness probe sees the window alive but the stall-expiry re-probe later in the same iteration sees it dead, that separate crash return skips try_resolve(). When SessionStart arrived before the rollout existed and no Stop followed, the now-present rollout is therefore omitted from the crashed result and its usage remains untracked; invoke the resolver before that window-death return as well.

Fixed in 2546e9a. The stall re-probe's crash return now calls try_resolve() too. The other exits are covered already: SessionEnd and the post-Stop returns run after the per-event resolve, and the over-budget exits need a sampled transcript. The new heartbeat retry also shrinks the window to at most one interval. Test: test_codex_stall_reprobe_death_resolves_at_exit, ablated.

Honor the supplied home environment for Codex fallback
When CODEX_HOME is unset, Path.home() reads the orchestrator process environment rather than the env mapping passed to this function. A profile or session that overrides HOME on POSIX or USERPROFILE on Windows therefore launches Codex writing to that pane-specific home, while fallback resolution searches the controller's home and returns no rollout. Derive the default home from the supplied mapping before falling back to the current process.
Useful? React with 👍 / 👎.

Fixed in 2546e9a. Without CODEX_HOME, codex_home now uses the home named in the passed env (USERPROFILE on Windows, HOME elsewhere, the same variables Path.home() reads), and falls back to the process home only when neither is set. Test: test_resolve_default_home_comes_from_the_passed_env, ablated.
- Retry the by-id rollout lookup on each heartbeat, so a rollout created after SessionStart arms the budget guard before Stop. - Resolve on the stall re-probe's window-death exit too. - Root the codex_home fallback at the passed env's HOME/USERPROFILE. - Name a rollout whose complete first line does not parse in `unread` (parent or sibling) instead of reading it as not-a-subagent; a first line still being written stays a silent, uncached miss. - List sibling day dirs directly (Path.glob swallows scan errors) and name an unlistable one in `unread` while still searching the others.

Retry rollout resolution before non-hook exits
When SessionStart arrives before Codex creates its rollout and the session times out, is hard-stopped, or stalls under a short grace before the next 30-second heartbeat, this retry is skipped because those terminal branches return first. The resulting SessionResult retains transcript_path=None, and unlike the Stop path it emits no transcript-unresolved crumb, so the engine records untracked usage even though the UUID-named rollout now exists; invoke try_resolve() before these terminal returns as is already done for both window-death paths.

Fixed in 8857192. The timeout exit and both hard-stop exits now call try_resolve() before returning. The stall exit now resolves before its liveness re-probe, so the crashed and the stalled branches both carry the rollout. That leaves no exit without a resolve: the hook-event exits run after the per-event resolve, and the over-budget exits need a sampled transcript. Tests: test_codex_non_hook_exits_resolve[timeout|abort-top|abort-after-wait] and test_codex_live_stall_resolves_at_exit. Ablating each call fails exactly its own case.

Record child files that disappear before the body read
If a sibling has already been classified as a child but is removed or becomes unstatable between _codex_meta() and this body read, _jsonl_entries() calls Path.is_file(), which suppresses the filesystem error and yields nothing. Consequently _codex_rollout_total() returns None, this except never appends to unread, and the child's usage is silently omitted from a total that appears complete; a known member that cannot be opened or statted should be recorded as unread.

Declining this one as an extreme edge case. The window is a few microseconds inside one _codex_session_total call, between reading a child's first line and reading its body, and Codex does not delete or move rollouts while a session runs. Faults that a real filesystem produces at this point (a read error, or a first line that does not parse) are already named in unread.
…775) The timeout, both hard-stop polls, and the live-window stall exit returned without retrying the by-id lookup, so a SessionStart that came before its rollout existed, followed by one of those exits before the next heartbeat, left the result untracked. Each now resolves first; the stall resolve moves ahead of the re-probe so the crashed and stalled branches both carry it.

Closes #775
Root cause (corrected)
The issue attributes
tokens: nullto codex's Stop payload having notranscript_path. That isn't the cause. Codex's hook schema (StopCommandInputandSessionStartCommandInput, v0.139 through HEAD) carriestranscript_path, which points at the live rollout. Locally, 521 of 521 codex SessionStart/Stop event files (codex 0.153.2–0.156.0, including 0.153.4) carry the path, and 319 of 319 completed codex sessions journal non-null tokens that equal codex's ownthreads.tokens_used. The "no transcript_path" evidence in the issue lists the probe report's top-level keys, not the payload's keys. The Windows symptom is still real. The relay (
bmad-loop relay <Event>→events._read_payload) ranjson.loadon a text stdin, which decodes with the locale code page (cp1252 withoutPYTHONUTF8). A decode error collapsed the whole payload to{}; a non-ASCII path came out garbled and pointed at a file that doesn't exist. Upstream openai/codex#23784 (truncated Stop stdin on Windows) is a second candidate. The exact Windows cause is not confirmed; it is filed as a follow-up below. This PR closes every one of those paths. There was also a separate bug on every platform: codex writes each subagent to its own rollout and leaves that usage out of the parent's
total_token_usage. The tally read only the parent, so subagent-heavy sessions were under-counted by half or more.Changes
5d46534a). A rollout's filename UUID is the hooksession_id, so a missing or unusable hooktranscript_pathis resolved under$CODEX_HOME/sessions(default~/.codex). The id is canonicalised as a UUID first, so it can't steer the glob. The resolution sticks against a later garbled hook path, and it also runs at window death. It is crumbed tosession-lifecycle.jsonl(transcript-resolved, ortranscript-unresolvedwhen nothing is found). The token budget guard now arms for these sessions.payload_error(de63b975). Both relay twins (events.pyanddata/bmad_loop_hook.py) decode stdin bytes withjson.loads, which detects UTF-8/16/32 and tolerates a BOM. An unreadable payload is taggedpayload_erroron the event (HookEvent.payload_error) and crumbed ashook-payload-unreadable. A valid payload's event is byte-identical to before, and completion handling is unchanged.217ddddd).tally_codex_rolloutadds each child's last cumulative totals. A child is a rollout in the parent's day dir or a later one (to cover midnight crossings), whose first-linesession_metanames the parent assession_idor whoseparent_thread_idchains into the tree. First-line facts are cached per path (bounded, oldest-first), so the budget guard's per-heartbeat tally re-reads only file bodies. Budget impact: codex totals and budgets can roughly double for subagent-heavy sessions, so budgets tuned on old figures may trip differently (CHANGELOG notes this).b4514da5). Any rollout the tally skips as unreadable is named onsession-endasusage_unread("<file name>: <ExcType>", no directory or message), so a lower-boundtokensis visible as one. That covers an unreadable sibling's first line or body, a failed sibling scan, and an unreadable parent first line. The list ridesTokenUsage.unread, which is excluded from equality,addandto_dict, so stored totals and--jsonare unchanged.bmad-loop diagnosedrops it to a presence flag because a rollout name embeds the session id. The budget guard's live samples ignore it, since a child still being written can tear a UTF-8 read. Errors on the parent rollout body propagate as before. Remaining edge: when nothing at all is readable, the tally still returns None and the skip is not recorded. Also: the stale codex.toml comment ("Codex has no SessionEnd hook event") is corrected, and the adapter authoring guide notes the by-id resolution.
