Since modelcontextprotocol#547 calls to an exited or killed stdio server fail at once, but isConnected() stays true.

php clients/php/repro/stdio-dead-server.php

https://github.com/modelcontextprotocol/php-sdk/blob/9591ad292788f717e8785b282edc2292850a98d4/src/Client/Transport/StdioTransport.php#L249 marks the request as connection lost, but nothing resets the client state. Same shape as the HTTP case with the 404.
