Document that OSV runs on pull requests and is required before merging into main.
Link the incident note from the public README and repository instructions.

Main test and OSV runs pass on 4413a92.
GitHub branch protection now requires scan / OSV-Scanner (CVE scan) from GitHub Actions, preserving all existing checks and strict branch freshness.
This documentation-only PR will exercise the passing OSV pull-request check.
