Why

public/.htaccess is preserved on upgrade, so every installation keeps the /uploads/ rules it was installed with. The 0.8.6-rc.1 heal only recognised the 0.7.71 – 0.8.5 private-uploads rule. On the demo installation, upgraded from an older version, it found nothing to change, and the file still served every existing upload straight from disk. The installation was safe only because its private folders happened to be empty.

On a real Apache, the shapes that shipped behave like this before any heal:

Shipped in Private-uploads rules Leaks
0.1 – 0.7.17 none every private file, at the root and in a subfolder
0.7.18 – 0.7.70 anchored on %{REQUEST_URI} every private file under a subfolder install
0.7.71 – 0.8.5 per-directory, without [NC] /uploads/Digital/... on a case-insensitive disk
0.8.6-rc.1 per-directory with [NC], before the public rule none

What changes

PrivateUploadsRewriteHeal now brings each shipped shape to the 0.8.6 pair: the per-directory private rule with [NC], followed by the public passthrough. It replaces only lines that are still exactly what was shipped, so rules an operator edited are left alone. It also refuses any result where the private rule would not come first.

The heal runs in PluginManager's once-per-version window, so it applies on the first request after any upgrade, whether in-app or manual.

A RewriteCond binds to the next RewriteRule across comments, blank lines and other directives, and a rule flagged C/chain makes the next one conditional. When a condition or a chain is still pending above the block, the heal leaves the rewrite rules alone, because rewriting them would move that condition onto a different rule. A declined heal still adds PDF and WAV to the no-gzip list, since that list does not depend on the rewrite rules.

Digital Library files across the upgrade

The upgrade from an older release now keeps every file uploaded on it. Files under public/uploads/digital move byte for byte to storage/digital at the same addresses. They stay open to anonymous visitors until the operator restricts them, and after that the case and dot-segment variants of the old path no longer reach the file.

Inactive bundled plugins

A bundled plugin that is installed but switched off only got its tables synced by onActivate. As a result, an install upgraded from 0.7.x kept ncip_transactions without prenotazione_id and its foreign key until someone switched NCIP on. The bundled plugin sync now brings the tables an inactive plugin already has to its version, without switching it on or registering hooks. A plugin that never created its tables, like an untouched Archives, is left alone.

This also covers the same-version path. The previous release's Updater runs the plugin sync itself, with its own PluginManager still in memory, so the version bump can be taken before the new code ever sees it.

Tests

  • tests/fixtures/htaccess-history/ contains the 13 distinct public/.htaccess files of the released tags.

  • tests/htaccess-heal-history-480.unit.php (195 checks) verifies, for each version:

    • the private rule with [NC] comes first;
    • no anchored REQUEST_URI rule is left;
    • every other directive is untouched;
    • running the heal twice changes nothing.

It also checks that edited or partial rules are left alone, and that a block under a pending condition or chain keeps its rewrite rules.

  • tests/htaccess-heal-apache-480.test.sh (104 checks) serves each version on a real Apache, before and after the heal, at the root and in a subfolder. Before the heal every older version leaks somewhere. After it, every private upload reaches index.php and covers stay static.

  • tests/digital-upgrade-480.spec.js uploads eBooks and audiobooks on the old release through the admin form, then checks them after the upgrade: the content, the anonymous downloads, and the access levels once the operator restricts them. It covers both the attachments list of 0.7.89+ and the single eBook and audiobook fields of older releases.

  • tests/plugin-schema-selfheal-fkcol.unit.php covers the inactive plugin sync, on a version bump and on the same version.

  • The full upgrade regression passes from 0.7.16, 0.7.68 and 0.8.5, which covers the three older shapes of the file.

  • scripts/ci-quality-local.sh passes.

I applied the same change by hand to the demo installation, with a backup in storage/backups. There, private files now go through the check, covers are still served statically, and the site works.

… one

public/.htaccess is preserved on upgrade, so an install keeps the shape it was installed with. The 0.8.6 heal only recognised the 0.7.71 – 0.8.5 private-uploads rule; older installs kept rules that serve private uploads straight from disk:

- up to 0.7.17 there is no private rule at all: every existing file under /uploads/, Digital Library files and archive documents included, is served as it is;
- 0.7.18 – 0.7.70 anchor the private rules on %{REQUEST_URI}, so under a subfolder they never match and existing private files are served as they are;
- 0.7.71 – 0.8.5 lack [NC], so /uploads/Digital/ skips the private rule on a case-insensitive disk.

PrivateUploadsRewriteHeal now brings each shape to the 0.8.6 pair (the per-directory private rule with [NC], then the public passthrough), replacing only lines that are still exactly what was shipped, and refuses any result where the private rule does not come first.

Tests: tests/fixtures/htaccess-history holds the 13 distinct shipped files. htaccess-heal-history-480.unit.php checks each healed file (private rule with [NC] first, no anchored REQUEST_URI rule left, every other directive untouched, idempotent) and that edited rules are left alone. htaccess-heal-apache-480.test.sh serves each one on a real Apache, before and after, at the root and in a subfolder: before the heal every format older than 0.8.6 leaks private files somewhere; after it none does and covers stay static (104 checks).

ℹ️ No blocking issues. Two rough edges in which files get healed, and how a declined heal is reported.

Reviewed changes

This review covers the single commit that extends the public/.htaccess heal from the 0.7.71 shape to every shipped shape.

  • PrivateUploadsRewriteHeal::apply() now rewrites shapes A (no private rule), B (anchored %{REQUEST_URI}) and C (per-directory without [NC]) into the 0.8.6 pair. A new safe() check rejects any result where the [NC] private rule does not come before the public /uploads/ rule.
  • History fixtures: there are 13 distinct shipped .htaccess files under tests/fixtures/htaccess-history/, whitelisted in .gitignore.
  • Tests: a pure unit test (htaccess-heal-history-480.unit.php) passes here with ALL 68 PASS. The real-Apache script (htaccess-heal-apache-480.test.sh) defaults to the Homebrew config path, so it skips on Linux and is not run in CI.

I also checked the healed output for 0.1.1, 0.7.21 and 0.7.71-rc.1 by hand. Only the /uploads/ rewrite lines and the no-gzip line change.

ℹ️ The failure messages and comments at both call sites still describe only the [NC] fix

PluginManager.php:2320 and Updater.php:4052 tell the operator to "add [NC] to the RewriteRule ^uploads/(digital|archives/documents|storage)/ line by hand". On a shape A or B install that line does not exist, so the advice does not apply. The comments above both calls (PluginManager.php:2314-2318 and Updater.php:4046-4050) also still say the heal only adds [NC].

Technical details
# Call-site comments and messages are stale for shapes A/B

## Affected sites
- app/Support/PluginManager.php:2314-2320: comment says the heal "adds [NC]"; the log message tells the operator to add [NC] by hand
- app/Support/Updater.php:4046-4052: same comment and same `debugLog` message

## Required outcome
- The comments describe the full heal (all shipped shapes moved to the 0.8.6 private + public pair).
- The manual-fix message tells the operator what the end state must be: `RewriteRule ^uploads/(digital|archives/documents|storage)/ index.php [NC,QSA,L]` before `RewriteRule ^uploads/ - [L]`, with no anchored `%{REQUEST_URI} ^/uploads/` rules left.

…clines

- REWRITE_C and the no-gzip pattern now end with a lookahead that accepts \r, so a 0.7.71 – 0.8.5 file saved on Windows gets [NC] too; the lines the heal adds keep the file's CRLF line ends.
- heal() returns false whenever the file still lets private uploads be served from disk, also when it declined a rule the operator edited. The once-per-version window and the Updater log it with the exact rule to add, so a declined heal is never silent.
- Tests: every shipped .htaccess converted to CRLF heals, stays CRLF and is idempotent; heal() returns true on a shipped file and false on an edited one.

✅ No new issues found. Both earlier findings are fixed cleanly.

Reviewed changes

This review covers e4dc83e, which makes CRLF files heal and makes a declined heal show up in the logs.

  • CRLF support: REWRITE_C and NOGZIP_OLD now end in (?=\r?$). block() takes an $eol, so the lines it adds use the file's own line endings.
  • Declined heals are reported: heal() now returns acceptable(), meaning the file is safe() or has no RewriteRule at all. An edited rule that the heal leaves alone now returns false. Both call sites log the exact rule needed and where it must go.
  • Tests: htaccess-heal-history-480.unit.php now checks a CRLF copy of every fixture: it heals, keeps CRLF throughout, and a second run changes nothing. It also covers heal()'s true/false paths on a temp file. I ran it here and got ALL 97 PASS. The DB-backed matrix test now expects false for an edited rule.

One thing to know about the new contract: safe() looks for the exact [NC,QSA,L] rule text. A custom configuration that is safe but written differently will therefore be logged as unsafe once per version window. Since that only produces a warning and the message also says "or restore the shipped file", it seems a reasonable trade-off.

Pullfrog  | View workflow run | Using claude-opus-5-5 | 𝕏

safe() accepted only the exact shipped text, so a custom file that is safe but written differently (flags in another order, extra flags or spaces, the long names nocase and last) was logged as leaking on every maintenance window. It now accepts any RewriteRule for the private folders whose flags include NC (or nocase) and L (or last), still required to come before the public /uploads/ rule.

ℹ️ No blocking issues. One small test gap is noted inline.

Reviewed changes

This review covers a370fe2, which makes safe() accept the private rule in any spelling.

  • Looser safe(): the check now matches any RewriteRule ^uploads/(digital|archives/documents|storage)/ index.php [...] line (any case, any whitespace) whose flags include NC/nocase and L/last. That line must still come before the first public /uploads/ rule. This removes the false "unsafe" warning I noted last time for custom files that are safe.
  • Tests: three new checks cover reordered flags, extra flags and missing NC. They also confirm apply() leaves a respelled file unchanged. I ran the suite here and got ALL 100 PASS.

✅ No new issues found. The test gap from the last review is fixed.

Reviewed changes

This review covers eb7de34, which changes only the test file.

  • Long flag names tested: the respelled rule is now [last,QSA,nocase], so the test now covers the LAST and NOCASE branches of safe().
  • Rule without L rejected: a new check confirms that safe() rejects [NC,QSA], because the public /uploads/ rule below it would still apply. I ran tests/htaccess-heal-history-480.unit.php here and got ALL 101 PASS.

Deep lane — correctness & security

(human-confirmed) ✓ fixed and verified (1bfc4f8)

Cross-cutting group G1: F003 + F006 — Both findings share one root cause in app/Support/PrivateUploadsRewriteHeal.php: neither apply() nor safe() checks for a RewriteCond pending above the block it matches. Add one helper, conditionPending(string $content, int $offset): bool, that scans backwards skipping blank and comment lines (CRLF-aware) and returns true only when the first other line is a RewriteCond. Use it in apply() to skip the REWRITE_C, PASSTHROUGH_A and PASSTHROUGH_B splices and the PRIVATE_B removal when a condition is pending, and in safe() to refuse a conditional NC+L private rule so heal() returns false and the callers log. Both halves must land together, with one combined set of tests in tests/htaccess-heal-history-480.unit.php (a RewriteCond above the anchor for 0.7.13, 0.7.21, 0.7.71 and a healed 0.8.6 file, with and without a comment or blank between), keeping shipped fixtures healing, idempotency, CRLF and the between-lines decline.

Details and fix proposals

F003 — Shape A heal splices the private block in place of the RewriteCond ^/uploads/ + RewriteRule ^ - [L] pair without checking the line above: an operator's RewriteCond directly above the matched pair is left behind and now binds to the inserted private rule, so the private rule only fires when the operator condition holds and every other private request falls through to RewriteRule ^uploads/ - [L] (served from disk) — while safe() still reports the file as safe.

File: app/Support/PrivateUploadsRewriteHeal.php:37-62
Score: 62 (moderate)

Evidence:

  • app/Support/PrivateUploadsRewriteHeal.php:37 — PASSTHROUGH_A anchors at the RewriteCond %{REQUEST_URI} ^/uploads/ [NC] line and never looks above it; the splice at :62 leaves an operator RewriteCond directly above in place.
  • app/Support/PrivateUploadsRewriteHeal.php:40,:60 — PASSTHROUGH_B is spliced the same way.
  • app/Support/PrivateUploadsRewriteHeal.php:161-168 — block() emits comments then PRIVATE_RULE; mod_rewrite ignores comments when binding conditions, so the leftover RewriteCond attaches to the inserted private rule and the public ^uploads/ - [L] loses its condition.
  • Reproduced shape A with RewriteCond %{HTTP_HOST} ^www\. [NC] above the pair: the condition ends up directly above the private rule and safe() returns true.
  • Reproduced shape B: the private rule becomes conditional on HTTP_HOST and other hosts get /uploads/digital/* served from disk — a regression from the previously unconditional PRIVATE_B pair.
  • safe() never checks for a gating RewriteCond, so heal() returns true and Updater.php:4051 / PluginManager.php:2319 log nothing.
  • Every shipped fixture has a comment above the anchor, so this needs an operator edit — the customised-file case the class docblock promises to respect; no test covers it.

Approach: Decline the splice when a RewriteCond is pending above the matched pair (nearest non-blank, non-comment line is a RewriteCond), and harden safe() to reject a private rule gated by a pending RewriteCond.

Files to modify:

  • app/Support/PrivateUploadsRewriteHeal.php — Add a conditionPending(string $content, int $offset): bool backward scan; skip the PASSTHROUGH_A, PASSTHROUGH_B and PRIVATE_B branches in apply() when it is true at the match offset, and require !conditionPending at the private-rule offset in safe(). (why: Prevents operator conditions from re-binding to the inserted private rule and makes heal() report the case.)
  • tests/htaccess-heal-history-480.unit.php — Add shape A/B cases with a RewriteCond above the anchor (also with a comment/blank in between) asserting apply() unchanged and heal() false, plus a safe() case with a condition above the private rule. (why: No fixture covers a pending condition.)

Verification:

  • php tests/htaccess-heal-history-480.unit.php
  • php tests/digital-access-matrix-480.unit.php
  • bash tests/htaccess-heal-apache-480.test.sh
  • grep -n conditionPending app/Support/PrivateUploadsRewriteHeal.php

Edge cases to preserve:

  • Shipped fixtures with a comment above the anchor still heal
  • A RewriteCond already bound to its own RewriteRule above is not pending
  • CRLF
  • Shape C operator condition is left in place but reported unsafe
  • Idempotency

Latest fix attempt (fixrun_20261020T112506Z84cb1f): fixed and verified

F006 — safe() accepts any NC+L private rule as protective without checking for RewriteCond lines directly above it, so a conditional private rule is reported safe — which also masks the condition-reattachment bugs of shapes A and B.

File: app/Support/PrivateUploadsRewriteHeal.php:122-131
Score: 62 (moderate)

Evidence:

  • app/Support/PrivateUploadsRewriteHeal.php:122-131 — safe() records the offset of the first NC+L private rule but never inspects the lines above it, so it cannot tell when a RewriteCond makes the rule conditional.
  • app/Support/PrivateUploadsRewriteHeal.php:37,40 — PASSTHROUGH_A/B match from the shipped RewriteCond; an operator RewriteCond directly above survives and, since block() starts with comments (Apache keeps conditions across comments), it binds to the new private rule.
  • Reproduced on the 0.7.13 fixture with RewriteCond %{HTTP_HOST} ^cdn\. [NC] above the passthrough: apply() changed, safe()=true, condition sits directly above the new private rule.
  • Reproduced on the 0.7.21 fixture: before the heal the private rule was unconditional; after, it only runs on cdn.* hosts and the public ^uploads/ - [L] serves /uploads/digital/* from disk on every other host — heal() reports success.
  • Reproduced on 0.7.71 (REWRITE_C keeps the condition) and on a hand-written 0.8.6 file with a condition above the private rule: safe()=true, heal() true.
  • Updater.php:4051 and PluginManager.php:2319 log only on false, so no warning is emitted.
  • tests/htaccess-heal-history-480.unit.php:84 only covers a condition BETWEEN the shipped lines, not above them.

Approach: Make safe() reject a private rule with an attached RewriteCond (scan back over blank/comment lines), and make apply() decline the A/B/C rewrites and the PRIVATE_B removal when a RewriteCond sits directly above the matched block.

Files to modify:

  • app/Support/PrivateUploadsRewriteHeal.php — Add a conditionAbove(string $content, int $offset): bool helper (backwards scan skipping blanks and comments, true if the first other line is a RewriteCond); use it in safe() to skip conditional private rules and in apply() to skip REWRITE_C, PASSTHROUGH_A, PASSTHROUGH_B and PRIVATE_B when a condition is attached. (why: Otherwise a conditional private rule is reported safe and the heal can move operator conditions onto other rules.)
  • tests/htaccess-heal-history-480.unit.php — Add cases with a RewriteCond above the shipped block for 0.7.13, 0.7.21 (passthrough and private), 0.7.71 and a healed 0.8.6 file. (why: Current suite covers only the between-lines case.)

Verification:

  • php tests/htaccess-heal-history-480.unit.php
  • php tests/digital-access-matrix-480.unit.php
  • bash tests/htaccess-heal-apache-480.test.sh
  • grep -n conditionAbove app/Support/PrivateUploadsRewriteHeal.php

Edge cases to preserve:

  • The E=HTTP_AUTHORIZATION RewriteRule above the shipped block is a RewriteRule, so those fixtures must still heal
  • Long comment blocks above the private rule are skipped
  • CRLF line ends
  • Long flag names and reordered flags stay safe
  • A condition between the shipped lines is still declined
  • Idempotency

Latest fix attempt (fixrun_20261020T112506Z84cb1f): fixed and verified

F008 — When the rewrite part of a heal is declined as unsafe, apply() returns the original content, discarding the independent no-gzip PDF/WAV fix too, so installs with an operator-edited /uploads/ rule never get the Range fix.

Promoted from disposition=uncertain / actionability=auto_fixable / score_phase4=45
Fix direction: In app/Support/PrivateUploadsRewriteHeal.php apply(), decide on the rewrite part first and apply the NOGZIP_OLD→NOGZIP_NEW replacement to whichever base is kept: the healed content when it is safe, or the original content when the rewrite is declined (rulesChanged && !safe). On the decline path every Rewrite* line must stay byte-identical to the input. Do NOT touch the REWRITE_C/PASSTHROUGH_A/PASSTHROUGH_B/PRIVATE_B regexes, conditionPending(), safe(), publicRuleBefore(), acceptable() or heal()'s true/false contract: heal() must still return false (so callers log) when the file is unsafe even if a no-gzip-only change was written. Keep CRLF handling. Add a case to tests/htaccess-heal-history-480.unit.php: a shipped fixture with the old no-gzip line plus an operator change that makes the heal decline (e.g. a RewriteCond above the shipped block); assert the Rewrite* lines are unchanged, the output has the pdf|…|wav no-gzip list, heal() returns false, and a second apply() is a no-op.

# Score Impact File Issue
F001 55 correctness app/Support/PrivateUploadsRewriteHeal.php:116-141 safe() matches the private rule's flags order/spelling-agnostically but the two 'public rule still ahead' checks are rigid literals ([L] and [NC] only), so a hand-edited public /uploads/ rule with a different flag spelling or order ([L,NC], [last]) that precedes the private rule goes unmatched and safe() returns true while private uploads stay exposed.

Phase 4 couldn't confirm decisively. Re-run /adamsreview:review if you suspect this deserves
further investigation with fresh context.

Below-gate findings (score < 45) that cluster in the same area — not worth surfacing individually, but dense enough that a human pass may catch something the pipeline filtered out.

# Score File Rough location Concern
F007 28 app/Support/PrivateUploadsRewriteHeal.php L54-71 The old code checked is_string() on each preg_replace result; the new code casts with (string), so a PCRE failure becomes '' and, on a file with no Rewrite lines, rulesChanged() is false and apply() returns '' — heal() would then overwrite public/.htaccess with an empty file.
F016 10 app/Support/PrivateUploadsRewriteHeal.php L54-56 REWRITE_C is applied with an unbounded preg_replace while the A/B branches replace once; a file containing the shape-C private rule more than once gets every copy rewritten, and safe() then judges only the first match.
F010 40 app/Support/PrivateUploadsRewriteHeal.php L80-93 heal() now returns false for any file still carrying RewriteRules that is not in the 0.8.6 shape, including custom-but-safe files safe()'s narrow patterns reject (e.g. [NC,QSA,END] or a [F,NC] deny rule); PluginManager calls heal() every maintenance window and logs SecureLogger::error on false, so those installs log a false leak warning repeatedly.
F015 35 app/Support/PrivateUploadsRewriteHeal.php L135-139 safe() only treats '^uploads/ - ...' passthroughs as public rules; a broader passthrough placed before the private rule (e.g. 'RewriteCond %{REQUEST_FILENAME} -f' + 'RewriteRule ^ - [L]', or an extension-based 'RewriteRule .(pdf
F002 15 app/Support/PrivateUploadsRewriteHeal.php L143-147 acceptable()'s fallback only checks for the total absence of any RewriteRule, not of an /uploads/ rule; a real public/.htaccess always has the front-controller RewriteRule, so the fallback never fires and acceptable() degenerates to safe() for every realistic file.

… block

mod_rewrite hands every queued RewriteCond to the next RewriteRule, whatever comments, blank lines or other directives sit in between. A condition the operator wrote above the shipped /uploads/ block therefore moved onto the private rule the heal inserted (shapes A and B), or stayed on the one shape C rewrote, so the private rule only applied when that condition matched and every other request reached the public passthrough. safe() still called the file safe, so neither the updater nor the maintenance window logged it.

apply() now leaves a block untouched when a RewriteCond is pending above it, and safe() no longer counts a conditional private rule as protective, so heal() returns false and the callers log the file. The history tests cover each shipped shape with a condition directly above the block and with a comment, a blank line or another directive in between.

✅ No new issues found. The heal now leaves a block alone when an operator's RewriteCond above it would bind to the spliced rule.

Reviewed changes

This review covers 80164ac, which makes the heal and safe() aware of a RewriteCond still waiting above a block.

  • Added conditionPending(): it walks back from the start of a block's line, and the first RewriteRule or RewriteCond it meets decides the answer. This matches mod_rewrite, where only a RewriteRule takes the pending conditions, and comments or other directives such as SetEnvIf, Options and RewriteBase do not. The docblock notes that the scan ignores <IfModule>/<If> nesting.
  • Made apply() skip blocks with a pending condition: shape C now goes through preg_replace_callback with PREG_OFFSET_CAPTURE and leaves untouched any match with a pending condition. The closure takes $out by value, so the offsets point into the original content. Shape B is skipped if either its passthrough or its private half has a pending condition. In the 0.7.21 fixture the private half follows the passthrough's RewriteRule ^ - [L], so normally nothing is pending there. Shape A is guarded the same way.
  • Tightened safe(): a private rule with a pending condition no longer counts as the guard, so a healed 0.8.6 file with an operator condition spliced in is reported as unsafe. The shipped E=HTTP_AUTHORIZATION rule still takes its own condition, and both the 0.8.6-rc.1 fixture and the healed output stay safe.
  • Added tests: the new cases put an operator condition above each A/B/C block in five ways (directly above, or with a comment, a blank line, SetEnvIf, or Options+RewriteBase in between). They also cover the negative cases, a lone directive and a condition already bound to its own rule. I ran tests/htaccess-heal-history-480.unit.php here and got ALL 148 PASS.

🔒 Security & Privacy | 🟡 Minor | ⚡ Quick win

  • RewriteRule ^uploads/ - [L];
  • RewriteCond %{REQUEST_URI} ^/uploads/ [NC].

Le forme seguenti non vengono quindi rilevate quando precedono la regola privata con [NC]:

  • RewriteRule ^uploads/ - [L,NC];
  • RewriteRule ^uploads - [L];
  • RewriteRule ^uploads/ - [L];
  • rewriterule ^uploads/ - [L].

In questi casi safe() restituisce true. heal() restituisce true tramite acceptable().

safe() only looked for the public /uploads/ rule as "RewriteRule ^uploads/ - [L]" or as the old "RewriteCond %{REQUEST_URI} ^/uploads/ [NC]" head. The same passthrough written as "^uploads/ - [L,NC]", "^uploads - [L]", "^/?uploads/.*$ - [END]", in lower case or with more spaces, above the private rule served private files from disk while safe() called the file safe, so neither the updater nor the maintenance window logged it.

safe() now takes every passthrough ("-") above the private rule that stops rewriting (L, last, END) or skips rules (S=n), and runs its pattern against the private paths, also in another case: without NC a pattern still reaches the file on a case-insensitive disk. Every spelling counts the same, a "^ - [L]" behind any RewriteCond included, while a passthrough without L, one limited to a public subfolder or a negated one stays safe. The shipped files only have E= rules there, which never stop, so none of them changes verdict.

@coderabbitai About the outside-diff note on safe() (the public /uploads/ rule only recognised in two exact spellings): fixed in e0ef520, more broadly than by widening the regex.

Instead of matching spellings, safe() now takes every passthrough (-) above the private rule that stops rewriting (L, last, END) or skips rules (S=n), and runs its pattern against the private paths (uploads/digital/…, uploads/Digital/…, uploads/archives/documents/…, uploads/storage/…), honouring NC. PCRE is the engine mod_rewrite uses, so the answer is Apache's: [L,NC], ^uploads - [L], ^/?uploads/.*$ - [END], lower case, extra spaces, a quoted pattern, a ^ - [L] behind any RewriteCond, and a lookahead that only excludes lower-case digital/ are all reported. A passthrough without L, one limited to a public subfolder, a negated pattern and the shipped E= rules stay safe.

The history test has 19 new checks (the 13 unsafe ones fail on the previous commit), and the Apache test still passes on every shipped .htaccess.

Reviewed changes

This review covers e0ef520, which changes how safe() finds a public /uploads/ rule above the private one.

  • Added publicRuleBefore(): it replaces the two exact-text public patterns. Every RewriteRule <pattern> - [...] above the private rule that stops (L/LAST/END) or skips (S=n) is now compiled and run against sample private paths. It honours NC and a leading !. A pattern PCRE cannot compile counts as a leak. This handles the respelled rules from the CodeRabbit comment on 80164ac ([L,NC], ^uploads, extra spaces, lowercase).
  • Added tests: 13 rules above the private one must make the file unsafe, and 6 must leave it safe (no L, a public subfolder, ^uploadsarchive/, a negated pattern, a lookahead excluding the private folders, E=). I ran tests/htaccess-heal-history-480.unit.php here and got ALL 168 PASS.

Comment thread app/Support/PrivateUploadsRewriteHeal.php Outdated
…private folders

Running each pattern against four sample paths only proved those paths: "RewriteRule \.epub$ - [L]" above the private rule matches none of them, so safe() called the file safe while Apache served every private EPUB from disk. The same went for a start in another case without NC ("^UPLOADS/Digital/"), a negated "!^uploads/" (which lets /Uploads/Digital/ through on a case-insensitive disk), and a literal start deeper inside a private folder.

A passthrough that stops rewriting or skips rules is now reported unless the pattern is anchored with ^ and its literal start, compared without case, leads away from every private folder ("^uploads/thumbs/", "^assets/"), with no "|" anywhere (in "^assets/|uploads/" the second branch has no anchor), and none of the sample private paths matches it either. Anything else, a lookahead included, cannot be proved without reasoning about the regex, so it is logged rather than trusted. The shipped files only have E= rules above the private rule, which never stop, so none of them changes verdict.

@coderabbitai Right, sample paths prove only those paths. Fixed in ad47c7d: a passthrough that stops (L/last/END) or skips (S=n) above the private rule is now reported unless it is provably harmless, i.e. anchored with ^, no | anywhere, and a literal start that leads away from every private folder, compared without case (so ^uploads/thumbs/ and ^assets/ pass). \.epub$ - [L], \.(pdf|mp3)$, ^(.*)$, ^UPLOADS/Digital/, !^uploads/, ^assets/|uploads/ and a lookahead are all reported, and are in the history test. The sample paths stay as an extra check only.

I test aggiunti coprono \.epub$, alternanze, pattern negati e prefissi dentro cartelle private. Non ho eseguito i test nel sandbox; questa verifica è statica.

✅ No new issues found. The extension-probe gap from the last review is closed, and the fix is stronger than the one I suggested.

Reviewed changes

This review covers ad47c7d. It changes safe() so that a stopping passthrough above the private rule counts as a leak unless the heal can prove it misses the private folders.

  • Added missesPrivateFolders(): a passthrough with L/LAST/END/S=n now passes only when it has no | anywhere and starts with ^ and a literal run. A literal character followed by ?, * or { is dropped from that run. The lowercased run must not be a prefix of uploads/digital/, uploads/archives/documents/ or uploads/storage/, and none of those may be a prefix of the run. Every probe path must also miss, compared case-insensitively. Probes now cover .epub, .mp3, .m4a, .jpg and .png too.
  • Updated the tests: they now report extension-keyed rules, quantifier-shortened starts, unanchored | branches, !^uploads/ and the lookahead exclusion. They also accept ^uploads/digitalx/ and ^assets/.*\.css$. I ran tests/htaccess-heal-history-480.unit.php here and got ALL 185 PASS.

I also tried a few edge cases by hand. ^uploads/digita[l]/, ^uploads/s\w* and ^uploads/(thumbs)/ are reported, and ^uploads/digitalz+ and ^assets/x{0,} are accepted. Some safe spellings are reported too, such as ^uploads\/thumbs\/. That only means an extra log warning, which is a reasonable price for a proof-only check.

Walkthrough decisions

rev_20261020T104246Z8517b2 · scope=qualifying · score_floor=45 · reviewer=fabiodalez-dev · ts=2026-10-20T12:53:21Z

Walking the Qualifying scope: of 2 non-auto-eligible finding(s), 1 promoted, 1 skipped.

Promoted findings will be picked up by the next /adamsreview:fix run via the human_confirmation bypass, regardless of its score threshold.

Promoted

  • F008 — When the rewrite part of a heal is declined as unsafe, apply() returns the original content, discarding the independent no-gzip PDF/WAV fix too. · option A (Decouple the no-gzip substitution from the rewrite-decline fallback)
    • Why: Apply the no-gzip update to whichever content is kept, so a declined rewrite no longer drops it; main applied it unconditionally.
    • Fix hint: decide on the rewrite part first, then apply NOGZIP_OLD→NOGZIP_NEW to the kept base; Rewrite* lines stay byte-identical on decline; heal() still returns false for an unsafe file; one regression test.
    • Prior: disposition=uncertain · score=45

Skipped

  • F001 — safe() recognised the public /uploads/ rule ahead of the private rule only in its shipped spellings.
    • Already addressed on this branch by e0ef520 and ad47c7d (every stopping passthrough above the private rule is now checked against the private paths).

Decisions log: this comment is append-only audit — it's never edited in place. Each /adamsreview:walkthrough run posts a fresh entry. Current state: see the main review comment and artifact.md.

apply() put PDF and WAV into the no-gzip list in the same pass as the rewrite heal, and when the rewritten rules turned out not to be safe (an operator passthrough such as "^uploads/ - [L,NC]" above the private rule) it returned the original file, dropping the no-gzip line with them. Such an install kept compressing PDF and WAV, so a Range request got the whole compressed body instead of the bytes on disk. Before this branch the no-gzip update was applied on every heal.

apply() now decides on the rewrite rules first and runs the no-gzip update on whichever content it keeps. A declined heal still leaves every rewrite rule as it was and heal() still returns false, so the caller logs the file; it only writes the no-gzip line, with a backup, and the next run writes nothing.

✅ No new issues found. A declined rewrite heal no longer drops the no-gzip update.

Reviewed changes

This review covers 1bfc4f8, which applies the no-gzip update separately from the rewrite-rule heal.

  • Reordered apply(): if the rewritten rules would not be safe(), $out goes back to the original content first. Only then does NOGZIP_OLD → NOGZIP_NEW run. A file with an operator passthrough above the private rule keeps every Rewrite* line but still gets PDF and WAV in the no-gzip list. The class docblock and the apply() docblock now say so.
  • Kept the heal() contract: on a declined heal the file now changes, so heal() takes one backup and writes it. It still returns false through acceptable(), so the callers still log it. The next run finds nothing to change and writes nothing.
  • Added tests: three declined shapes are covered (0.7.71 with ^uploads/ - [L,NC] above, 0.7.71 with \.pdf$ - [L] above, and 0.7.13 with ^uploads/ - [L] above). Each checks that the rules stay the same, the no-gzip line is updated, safe() stays false and a second pass changes nothing. An on-disk check confirms one backup, false on both runs, and no write on the second run. I ran tests/htaccess-heal-history-480.unit.php here and got ALL 189 PASS.

Comment thread app/Support/PrivateUploadsRewriteHeal.php
A RewriteRule with C (chain) runs the next rule only when it matched itself, so "RewriteRule ^uploads/thumbs/ - [C]" right above the private rule skips it for every private path, whatever its substitution. conditionPending() stopped at the first RewriteRule and called the queue taken, so safe() counted the chained private rule as protective and the heal could rewrite a shape C rule into the chain.

The backward scan now treats a RewriteRule carrying C or chain like a pending RewriteCond: the private rule below it is not counted as safe, and apply() leaves that block as it is so heal() reports the file. A cookie flag such as CO= is no chain and stays safe.

✅ No new issues found. A rule chained into the private rule now counts as a condition, which covers the CodeRabbit comment on 1bfc4f8.

Reviewed changes

This review covers 572ad97, which makes conditionPending() treat a chained rule above the private rule as conditional.

  • Extended conditionPending(): when the first RewriteRule found walking back has C or CHAIN in its trailing [flags] (any case, trimmed), a condition is now pending. As a result, safe() no longer counts a chained private rule as the guard, and apply() no longer rewrites a block that sits inside a chain. Lines are still split on \r?\n, so CRLF files get the same check. None of the shipped fixtures has a [C] rule, so the heal output does not change for them.
  • Added tests: three chained spellings above the healed 0.7.71 private rule are reported ([C], [chain], [QSA, c]). The 0.7.71 C shape is left alone and reported when an ^assets/ - [C] rule sits right above it. A CO= cookie flag is not mistaken for a chain. I ran tests/htaccess-heal-history-480.unit.php here and got ALL 195 PASS.

tests/digital-upgrade-480.spec.js runs twice against the same sandbox install. Before the upgrade the admin uploads two PDFs and an audiobook through the old book form and an anonymous visitor downloads them, Range requests included. After it, the same files are in storage/digital byte for byte at the same addresses, still open to everyone while the default is left as it was, and once the admin limits one to signed-in readers and one to staff the visitor is sent to sign in or refused, another case of the folder name or a dot segment does not get around the check, the reader opens only theirs and the admin both.

digital-access-480.spec.js takes the install root from E2E_INSTALL_ROOT, so the same suite can run against the upgraded sandbox.

✅ No new issues found. This commit only adds and adjusts tests, and the heal code is unchanged.

Reviewed changes

This review covers f96e474, which adds an end-to-end upgrade test for Digital Library files uploaded on an older release.

  • Added tests/digital-upgrade-480.spec.js: a Playwright spec that runs in two phases, selected by E2E_UPGRADE_PHASE, with the state between them kept in a JSON file.
    • before: the admin uploads two PDFs and an MP3 through the book form. The files land in public/uploads/digital, and an anonymous visitor gets 200 and 206 responses.
    • after: the files are in storage/digital, byte for byte, at the same URLs. Under the inherited default they still open. Then the admin sets one file to users and one to staff. An anonymous visitor now gets a 302 to sign in for the first and a 403 for the second, and the /Digital/ and /./ spellings of the URL do not serve the file either. A signed-in reader can open the users file but not the staff one. The admin can open both.
  • Adjusted tests/digital-access-480.spec.js: ROOT now honours E2E_INSTALL_ROOT, like the other specs. The check on the old folder's .htaccess now ignores a Require all denied inside <FilesMatch>, which the pre-0.8.6 plugin uses to deny scripts only.

Checks I ran:

  • Every selector and form field the spec uses exists in storage/plugins/digital-library/views/.
  • The columns of the inserted utenti row match installer/database/schema.sql.
  • [data-attachment-row] was added on 2026-10-04, before 0.8.4. The "old release" in the before phase therefore needs to be at least that recent.
  • node --check passes on both specs, and tests/htaccess-heal-history-480.unit.php still reports ALL 195 PASS.

The new spec is not run by any CI workflow, only by the local, gitignored scripts/reinstall-test.sh Test B. That matches the existing upgrade specs.

…89 in the upgrade test

Before the attachment list of 0.7.89 the book form had one eBook and one audiobook field, and the edit page lived at /admin/libri/modifica/N up to 0.7.17. The upgrade test now uploads through whichever form the old release has, gives each file a role (open, readers, staff) instead of a label, finds the rows of the upgraded form by address, and also tries /UPLOADS/ on the protected files.
…on changes

When a bundled plugin's version on disk moved past the one in the database, only an active plugin had its schema brought up to date (onActivate runs ensureSchema). A plugin that is registered but switched off kept its tables as they were. An install upgraded from 0.7.x keeps ncip-server registered and off, with the ncip_transactions of that time, so it lacked prenotazione_id and its foreign key until someone switched NCIP on, and its schema differed from a fresh install's.

The version bump now runs ensureSchema for an inactive plugin whose declared tables already exist and miss a column or foreign key. It registers no hook and leaves the plugin off. A plugin whose tables were never created, like an Archives nobody activated, is left alone, since activating it creates them. The check uses is_callable because most plugin wrappers forward these methods through __call.

plugin-schema-selfheal-fkcol.unit.php covers both cases: the inactive ncip-server gets the column and foreign key at the bump and stays off without hooks, and an inactive Archives without tables gets none.
…it without them

Some 0.7.x installers left digital-library active with only its routes hook, so the book form had no uploader and the upgrade test could not upload anything on 0.7.68. The test now switches the plugin off and on from the plugins page when book.form.digital_fields is missing, as an admin would, and checks the hook is there before uploading.
…y taken

The previous release's Updater runs the bundled plugin sync itself, with its own PluginManager still in memory. On an install upgraded from 0.7.16 that old code takes NCIP's 1.0.0 to 1.0.5 bump, so the new code finds the plugin already at the disk version and never adds ncip_transactions.prenotazione_id. The inactive-plugin schema sync now also runs on the same-version path. It runs once per release, and only reads until a column or foreign key is actually missing.

Reviewed changes

This review covers 62a5144..a07aba1. These commits extend the upgrade E2E spec to older book forms, and add a schema sync for bundled plugins that are switched off.

  • Added syncInactivePluginSchema(): on a version bump, an inactive bundled plugin now gets its ensureSchema() run when all its declared tables exist and a declared column or foreign key is missing. A new same-version elseif branch does the same, for installs where the previous release's Updater already took the bump. A plugin with no tables (e.g. Archives that was never switched on) is left alone.
  • Added DB tests: scenario C in tests/plugin-schema-selfheal-fkcol.unit.php checks that an inactive NCIP gets prenotazione_id and its FK back, both on a bump and at the same version, and stays off with no hooks. Scenario D checks that Archives gets no table.
  • Extended tests/digital-upgrade-480.spec.js: it now handles the single file_url/audio_url fields of releases before 0.7.89 and the old /admin/libri/modifica/N route. It switches Digital Library off and on again when book.form.digital_fields is missing. Files are now tracked by role (open/readers/staff), and the spec adds an /UPLOADS/ variant to the bypass checks.

I checked HookManager::loadHooks(), which joins on p.is_active = 1. So when DigitalLibraryPlugin::setPluginId() writes hook rows during this sync for an inactive Digital Library, those hooks never fire. deactivatePlugin() already does the same thing. No bundled wrapper exposes expectedTables only through __call, so the is_callable/method_exists split does not hide any table today. tests/htaccess-heal-history-480.unit.php still reports ALL 195 PASS, and php -l and node --check are clean. I could not run the DB-backed test here.

Comment thread app/Support/PluginManager.php Outdated
The comment said the same-version sync of an inactive bundled plugin runs once per release. autoRegisterBundledPlugins() runs on every maintenance window (the 900-second plugins_maintenance_* marker per version stamp) and on every getAllPlugins() call, so on each load of the admin plugins page. The check stays read-only until something is missing; the comment now says when it runs, so nobody relies on a once-per-release guarantee.
…ccepts

The regular browser shards run every spec, and digital-upgrade-480.spec.js has nothing to do there without E2E_UPGRADE_PHASE. Its single skip reason, "E2E credentials and E2E_UPGRADE_PHASE required", matched the audit's "required" pattern, so all 8 tests counted as hidden infrastructure failures and failed shard 2.

The missing phase now skips with its own reason, like the 412 upgrade spec, while missing credentials keep a reason the audit still flags.

✅ No new issues found. The comment I flagged about how often the pass runs is now accurate, and the upgrade spec no longer trips the CI skip audit.

Reviewed changes

This review covers 5789146 and 0cbbed3, the two commits after a07aba1.

  • Corrected the cadence comment: the comment above syncInactivePluginSchema() in PluginManager.php now says what actually happens. The pass runs on every 900 s plugins_maintenance_* window and on every getAllPlugins() call. That matches loadActivePlugins() (QueryCache::set($maintenanceKey, 1, 900)).
  • Split the upgrade spec's skip: tests/digital-upgrade-480.spec.js is not listed as excluded or dedicated in scripts/ci-playwright-policy.js, so it runs in the deep-regression shards, where no phase is set. The old reason (… E2E_UPGRADE_PHASE required) matched the required term in fatalReason in scripts/ci-audit-playwright-results.js, so the audit failed it as a suspicious skip. The new phase reason matches none of the fatal terms (it has no set e2e_ either), and it is registered first. If both conditions hold, the audit reads that first skip annotation. The credentials reason, which still says required, can only fire when a phase is set, which happens only in local upgrade runs.

php -l and node --check pass on both changed files.

…ng hooks

syncInactivePluginSchema() built its instance through instantiatePlugin(), which always calls setPluginId(). digital-library, open-library, goodlib and scraping-pro register their hooks there whenever they have none, and digital-library also moves its files, so syncing the schema of a plugin the admin switched off gave it hook rows. instantiatePlugin() now takes $setPluginId (true by default) and the schema-only path passes false; no ensureSchema() reads the plugin id. Scenario E covers it with an inactive digital-library, and fails on the previous commit.

Scenario C now drops prenotazione_id together with whatever foreign key holds it under its actual name, and registers a restore, so a failure halfway puts NCIP's activation state, version and hooks back.

The upgrade spec sends the /Digital/, /./ and /UPLOADS/ variants through curl --path-as-is: Playwright's request.get parses the URL and drops the "/./" segment, so that variant only repeated the plain request.

✅ No new issues found. A plugin that is switched off no longer gets hook rows or a file move from the schema sync.

Reviewed changes

This review covers 233565b, the only commit after 0cbbed3. It addresses CodeRabbit's three inline comments on a07aba1.

  • Schema-only instance: instantiatePlugin() now takes bool $setPluginId = true, and syncInactivePluginSchema() passes false. A switched-off Digital Library therefore no longer runs ensureHooksRegistered() or ensureStorageMigrated() during the sync. I checked that no bundled ensureSchema() reads $this->pluginId. Digital Library's version only adds libri columns and digital_library_files. Every other caller keeps the default.
  • Exact-path bypass checks: the /Digital/, /./ and /UPLOADS/ checks in digital-upgrade-480.spec.js now go through curl --path-as-is, so the /./ request is no longer collapsed into the plain URL before it is sent.
  • Sturdier DB test: scenario C now drops prenotazione_id by the foreign key's real name, found in information_schema. It also registers a restore for is_active and the version. A new scenario E confirms that an inactive Digital Library at an older version gets libri.audio_url back with zero hooks and stays off. Its restore runs before scenario B's, and B's restore re-adds the column values from zz_sfc_dl_bak and the original version.

php -l passes on both PHP files and node --check passes on the spec. I could not run the DB-backed test here.

…g in END

apply() cast every preg_* result to a string. When PCRE gives up (backtrack or JIT
limits) those calls return null, so apply() returned '' and heal() wrote an empty
public/.htaccess, then called it acceptable because it holds no RewriteRule: the site
lost its front controller and every protection with it (review F007). A null now keeps
the file as it is, and the next maintenance window tries again.

safe() took a private rule as protective only with L or last. END stops rewriting too, so
a custom file with [NC,QSA,END] was reported as leaking on every maintenance window (part
of F010).

tests/htaccess-heal-history-480.unit.php runs apply() and heal() with pcre.backtrack_limit=1
and checks the file on disk is unchanged, and checks the END spelling; the three checks fail
on the previous commit (the on-disk file was emptied). ALL 198 PASS, and
digital-access-matrix-480.unit.php still passes.

Outcome of the "Polish — below threshold" findings of review rev_20261020T104246Z8517b2, checked against the current head:

  • F007 (PCRE failure → empty file): fixed in 275bf2d. Confirmed: with pcre.backtrack_limit=1, apply() returned '', and heal() wrote an empty public/.htaccess and returned true. A null from any preg_* call now keeps the original content. tests/htaccess-heal-history-480.unit.php runs apply() and heal() under that limit and checks the file on disk is unchanged. The test fails on the previous commit.
  • F010 (custom but safe file logged as leaking): fixed for END in 275bf2d. safe() now also takes a private rule ending in END, not only L/last. A [F,NC] deny rule with another substitution is still reported. That stays conservative on purpose, since the heal only vouches for the shape it knows.
  • F015 (broader passthrough before the private rule): already fixed by ad47c7d. publicRuleBefore() reports every stopping or skipping passthrough unless its anchored literal start leads away from every private folder. RewriteCond -f + RewriteRule ^ - [L] and \.(pdf|epub)$ - [L] are both reported.
  • F016 (REWRITE_C replaced unbounded): no change. Rewriting every copy of the shape-C private rule to [NC,QSA,L] only makes each copy protective, and safe() still checks the first stopping one against everything above it.
  • F002 (acceptable() fallback rarely fires): no change. It is meant only for a file with no RewriteRule at all (for example a leftover file on nginx), where the heal has nothing to act on. For every Apache file acceptable() is safe(), as intended.

ALL 198 PASS on the history test, and digital-access-matrix-480.unit.php passes against a local MariaDB.

ℹ️ No blocking issues. apply() no longer writes an empty file when PCRE gives up, but heal() still reports that case as safe. Details are below, plus one small note inline.

Reviewed changes

This review covers 275bf2d, the only commit after 233565b.

  • PCRE failure keeps the file: apply() no longer casts preg_replace* results to string. When any of them returns null, it returns the original $content, so heal() can no longer write an empty public/.htaccess.
  • END counts as a stop flag: safe() now accepts a private rule with END as well as L/LAST, so a custom but safe [NC,QSA,END] file is no longer reported.
  • Tests: the new tests cover the END private rule, and with pcre.backtrack_limit=1 and JIT off they check that apply() and the on-disk heal() leave the file unchanged. I ran tests/htaccess-heal-history-480.unit.php here and got ALL 198 PASS.

ℹ️ heal() still returns true for an unhealed file when PCRE gives up

The new comment says the old bug was that heal() "called acceptable because it has no RewriteRule". That second half is still there. In acceptable(), preg_match(...) !== 1 treats a PCRE false as "no RewriteRule". I tried it here: with pcre.backtrack_limit=1 and JIT off, heal() on the untouched 0.7.71 fixture returns true. Neither caller logs anything, even though the file still leaks. CodeRabbit's outside-diff comment on PrivateUploadsRewriteHeal.php:226 is right about this. The new test calls heal() without checking its return value, so it does not catch the problem.

Technical details
# acceptable() treats a PCRE failure as "no RewriteRule"

## Affected sites
- app/Support/PrivateUploadsRewriteHeal.php:226 — `preg_match('/^\s*RewriteRule\b/m', $content) !== 1` is true when preg_match returns false
- tests/htaccess-heal-history-480.unit.php:262 — `Heal::heal(...)` return value is not asserted under the PCRE limits

## Required outcome
- When PCRE fails, `heal()` returns false (so PluginManager/Updater log it) instead of true.
- The PCRE-limit test asserts `!Heal::heal($tmpFile, ...)`.

## Suggested approach
- Use `=== 0` in `acceptable()`.

… up after the match

acceptable() now treats a preg_match error as "has rewrite rules", so heal()
returns false instead of calling the file safe. The PCRE test asserts that,
and adds a 0.7.71 file whose rule is recognised but whose replace gives up on
a later line: apply() keeps the original and heal() leaves the file on disk.
The --path-as-is curl in the upgrade spec gets --max-time and an exec timeout.

ℹ️ The acceptable() fix is good, but the ?? $out fix described on the resolved thread at line 96 is not on this branch. Details inline.

Reviewed changes

This review covers 23c9df7, the only commit after 275bf2d.

  • PCRE failure is reported: acceptable() now uses === 0, so when preg_match returns false, heal() returns false and the callers log it. The PCRE-limit test now asserts !Heal::heal(...). This resolves the body note from the previous review and CodeRabbit's outside-diff comment.
  • Give-up-after-match test: a 0.7.71 file with a near-miss [QSA,L] line padded with 2000 blanks, run at backtrack_limit=1000. The recognition preg_match succeeds and preg_replace_callback returns null, so the test reaches the $out === null guard.
  • curl timeouts: getPathAsIs() now passes --max-time 15 to curl and timeout: 20000 to execFileSync.

tests/htaccess-heal-history-480.unit.php prints ALL 202 PASS here. The author's reply mentions 203. php -l and node --check both pass.

✅ No new issues found. a88b58c contains the ?? $out fallback, as the author said on the line 96 thread.

Reviewed changes

This review covers a88b58c, the only commit after 23c9df7.

  • The no-gzip fallback keeps the rewrite heal: if the NOGZIP_OLD replace gives up, apply() now falls back to $out instead of $content. By that line, $out has already passed safe() or been reset to $content, so the fallback cannot return an unchecked rewrite.
  • Added a give-up test for the no-gzip replace only: the test appends to the 0.7.71 fixture an old no-gzip line with 2000 trailing blanks. I ran that input on its own at backtrack_limit=1000 with JIT off: preg_replace(NOGZIP_OLD, …) returns null with "Backtrack limit exhausted". So the test really exercises the fallback, and it would have failed with the old ?? $content.

tests/htaccess-heal-history-480.unit.php prints ALL 203 PASS here, and php -l passes.
