Phase 0/1 groundwork for #99 and #105. See docs/plans/2026-10-10-open-issues-ultraplan.adoc.

  • Adds the ultraplan.
  • Adds docs/governance-exemptions.adoc. The live Actions-policy check is recorded as not evaluated without HYPATIA_SCAN_PAT, with a compensating control and review date 2026-11-10. Nothing is muted.
  • Adds a manual workflow_dispatch job to lock-sync-gate.yml that regenerates actions.lock with gh actions-lock. It adds no uses: lines, so the lock stays in sync.

No pin change in this PR. The governance pin bump follows in a separate PR once this regeneration path is on main.

Refs #99 #105 #108

…k regeneration job

- docs/plans: ultraplan for the three #99 checks and #105 (decisions, sequencing).
- docs/governance-exemptions.adoc: the live Actions-policy check is recorded as
not evaluated without HYPATIA_SCAN_PAT, with compensating control and review
date 2026-11-10. No check is muted.
- lock-sync-gate.yml: add a manual workflow_dispatch job that regenerates
actions.lock with gh actions-lock on the dispatched branch. No uses: lines are
added, so the lock is unaffected.

📝 Summary

Summary by CodeRabbit

  • New Features
    • Added a manual workflow option to regenerate and verify workflow locks, committing updates when changes are found.
  • Documentation
    • Added a record explaining the governance check’s credential-dependent status and the safeguards that remain active.
    • Added a draft plan covering reported issues, candidate governance versions, verification steps and decisions still awaiting review.

Walkthrough

The PR adds a manually dispatched workflow for regenerating and pushing workflow lock changes. It also adds a governance-exemption record and a draft plan that documents issue findings, owner decisions, and proposed follow-up work.

Changes

Governance and lock maintenance

Layer / File(s) Summary
Manual lock regeneration
.github/workflows/lock-sync-gate.yml
Manual dispatch runs the pinned gh-actions-lock extension against the dispatched branch. The workflow commits and pushes workflow changes when present, and exits without committing when none exist.
Issue findings and governance status
docs/plans/2026-10-10-open-issues-ultraplan.adoc, docs/governance-exemptions.adoc
The draft plan records findings on issues #99, #105 and #108, candidate pins, gate results, and UUID counts. The exemption record describes the credential-dependent Live Actions-policy check and the separate active Allowlist Preflight gate.
Constraints and owner decisions
docs/plans/2026-10-10-open-issues-ultraplan.adoc
The draft plan lists binding constraints and six owner decisions for the proposed work.
Proposed execution and follow-up
docs/plans/2026-10-10-open-issues-ultraplan.adoc
The draft plan sets out proposed phases, verification and closure criteria, a separate compliance track, recorded risks, proposed issue determinations, and actions not taken.

The new manual lock-regeneration job pushes unsigned commits, which conflicts with the repository signing policy and may be rejected or block squash merging. Resolve this before relying on the job. The other changes are documentation and minor fixes.

Security Architecture Review

Security architecture risk: 🔵 Low · up to 73de6

Manual maintenance now carries repository-write authority and publishes local bot commits through a path that conflicts with the documented signing policy. Exposure is bounded by operator access and repository protections, but those protections have not been confirmed for every selectable branch.

Retained concerns

  • Low · security · inferred: The new manual path runs selected-ref workflow code with repository-content write permission, without an explicit trusted-ref restriction. This introduces privileged runtime execution into a previously read-only workflow. Actual misuse remains conditional on branch influence, authorized dispatch, and effective token and branch controls; it is not demonstrated as an escalation beyond existing workflow-writer authority.
  • Low · security · observed: The added job creates and directly pushes local bot commits instead of using the documented API or signed-push path, and configures no signing mechanism in the workflow. This conflicts with the repository's commit-provenance contract. Whether protections reject the publication or permit a policy-inconsistent commit is unknown.
Security review details

Security Blast Radius

  • inferred — The visible publication path affects a selected branch in this repository and stages workflow-directory changes. The requested token authority is repository-content write, broader than the intended lock update. Successful abuse could affect repository integrity and potentially downstream automation, but cross-repository, tenant-wide, or Administration authority is not demonstrated.

Security Findings and Attack Paths

  • inferred — The retained static finding concerns selected-ref execution under the new write-token job. Its attack path requires influence over executable workflow content and an authorized dispatch, followed by effective permission to mutate repository contents. Workflow writers could already author an equivalent capability before this PR; the introduced runtime path is supported, but a new privilege escalation or protection bypass is not established.

Trust Boundaries and Controls

  • observed — The write job is guarded by workflow_dispatch rather than pull_request. Credentials are supplied through environment variables and command-local HTTP headers rather than embedded in the remote URL. The implementation nevertheless uses local bot commit/push instead of the documented API-based provenance boundary. Effective dispatch authorization and branch enforcement are not supplied.

Resilience and Maintainability Implications

  • inferred — Fail-fast generation and verification, no-diff repetition, and non-force publication contain ordinary failure and concurrency effects. Live signed-commit enforcement could reject the new repair path; absent enforcement, the documented provenance control is not implemented by that path. Neither outcome should be assumed without the effective configuration.

Hardening Proposals

  • proposed — Use the repository-approved publication mechanism with verified commit-signing support, restrict and validate the intended branch targets, and constrain publication to reviewed lock outputs. Confirm effective dispatch authorization, token permissions, and branch rules before relying on this maintenance path.

28-29: 📐 Maintainability & Code Quality

The document states that HYPATIA_SCAN_PAT is absent, but the supplied evidence does not include the plan response or the caller workflow's secret mapping. The claim cannot be decided without those sources or owner confirmation.

=== D2. Live Actions-policy check (#99 Allowlist half, #105 token half)

* *A. Provision `HYPATIA_SCAN_PAT`* as a repository secret: a token that can read the repository's Administration endpoints. Owner action only. The live check then evaluates the policy. It still exits 0 with a notice if the token cannot read Administration, so the owner should accept that case too. This is the elegant target.

Pass the PAT through the caller workflow. The supplied .github/workflows/governance.yml caller has no secrets: map or secrets: inherit. GitHub does not automatically pass repository secrets to reusable workflows, so provisioning HYPATIA_SCAN_PAT alone leaves the called workflow's secret empty and the check reports “not evaluated”. Review comment at @docs/plans/2026-10-10-open-issues-ultraplan.adoc at line 273:
Update docs/plans/2026-10-10-open-issues-ultraplan.adoc, lines 273-273, to
require an explicit HYPATIA_SCAN_PAT secret mapping in the caller workflow
before the live check is considered evaluated. Update
docs/governance-exemptions.adoc, lines 43-44, to require the same caller mapping
before confirming evaluation and removing the exemption.
