Summary

Review all 12 active knowledge pages against fetched default-branch revision a315092e0a84160aaf336091e6e547445208324e in a fresh worktree, following docs/README.md, docs/maintenance.md, and CONTRIBUTING.md. Refresh each page's review timestamp after checking its implementation, callers, tests, tooling, navigation, and relevant external references.

Correct the README's state-ownership summary and shorten detailed frame-failure notes into lifecycle guidance with links to implementation and tests. No behavior, test, fixture, archive, or legal-content changes.

Verify that issue #199 and PRs #200, #266, and #271 remain open; active debt remains unresolved. The prior inspection (#351 / #352) is merged.

Validation

• Documentation validation: 29 passed, including maintained scope, review metadata, freshness, local links, anchors and navigation.
• just test: 1,063 passed on Python 3.12.16, with two existing pandas deprecation warnings.
• just check, uv run prek run --all-files, and git diff --check: passed.
• Timestamp automation tests: 20 passed; PR checks: 21 passed; example configuration tests: 6 passed. Both Slim example --help commands passed.
• Focused frame observation/failure/layer and model dispatch/accumulator checks: 80 passed.
• A fresh context-free reviewer independently checked all 12 pages, preserved the precise Exception qualification in failure ordering, and passed 33 documentation/frame-failure tests.
• A separate fresh final naming pass completed without renames.

GitHub CI passed, including Python 3.12/3.13 tests, Check, PR title validation, document timestamp automation, CodeQL and CLA. No new CLA signature action was required.

Python 3.13, performance benchmarks, and live model/daemon/Redis integrations were not run locally.
