Early returns in the action handler, like a missing action set or an empty select, skipped the "no response" fallback, so Discord showed "didn't respond in time". The fallback now runs in HandleActionInteraction for every path. A missing action set in a server gets an ephemeral hint to re-send the message, since the editor stores a set for every button and option; in DMs it's acked silently because DM components are never hooked up to actions.
