Cancel the reader on the read-error path.

When ctrl.enqueue(value) throws, reader still locks transformed, so transformed.cancel() rejects with TypeError. The empty catch suppresses that error. The finally block then releases the lock and clears this.reader without cancelling transformed.

disconnect() has no later path that cancels transformed: it can cancel this.reader only while it exists, and _fromDevice.cancel() does not cancel the locked pipe. The pipe can therefore keep portReadable locked, causing connection.close() to fail.

reader.cancel() and readLoopPromise only cover the transformed reader. The pipeThrough() operation can still hold portReadable when disconnect() calls connection.close(). Web Serial can reject that close while the raw stream is locked, leaving the port open.
