On stdio, as soon as a second tool call suspends its fiber (progress, log, elicit, sample), the first one never answers - the transport only kept the last fiber.

Fixes chr-hertel#57

Concurrent tool calls that report progress or elicit replaced each other's fiber, so only the last one finished. Protocol tracks the awaited request per fiber, and the transport callbacks get the fiber.
