feat: add consultations and context reporting

• Read-only requests — questions, advice and brainstorming — are handled as consultations: answered from a refreshed main branch with no ticket, workspace or request file, and recorded only when the exchange produced a decision. Threads a human opened and tagged the bot into are handled the same way, and replies stay concise.
• alcode records the context-window size of each run, alcode status reports it, and the delegation guide uses it to decide whether planning continues in the spec's session. alcode usage is renamed to alcode quota.
• The setup guide treats the AlignFirst protocols, the command skills, docmap and workspace as independent adoptions, and routes the project instructions section to whichever of them the project installed.
• Test scenarios are renumbered to a contiguous A01–A23, with new cases for consultations on an existing branch and for human-created threads, and coverage of the recorded context size.
