Fixes the two core unit failures in publish run 41149399275.

• plugin/provider.ts: the fork still listed DynamicProviderPlugin in ProviderPlugins (pre). Upstream fix(core): run dynamic provider npm fallback after external sdk hooks anomalyco/opencode#60380 moved it to post so external aisdk.sdk hooks run before the npm fallback; the duplicate ID kept the pre copy, so the fallback ran first. Drop the fork's pre registration. Fixes PluginSupervisor > runs SDK plugin aisdk.sdk hooks before the dynamic npm fallback.
• tool-registry test: since the v2.2.x sync the code-mode runtime only exposes code-mode tools, so a codemode: false tool is no longer reachable from execute at all. Retarget the fork's cache test at a code-mode tool so it still proves a later denial is not bypassed by the cached runtime.

Local: core typecheck clean; CI-equivalent bun turbo test 34/34 tasks, 0 fails.
