docs(plans): fix the permission policy's evidence path and hook list - #232

The Evidence section pointed at a session scratchpad that no longer exists. It now points at ~/.local/state/permission-policy/denials-v2/. Add the copy gate's redirect-target false positive to the Hooks table and the hook follow-ups: _commit_args_ok skips a spaced `>` but denies the next word as a pathspec. Mark the EnterWorktree path re-entry fix done (claude-config#796).

Follow-up to #231. These are corrections to the permission policy plan only. No hook or script changes.
