docs: align spec artifacts to use "overwritten" action terminology - #80

ScaffoldDCP() returns "overwritten" (consistent with Scaffold()), but three spec documents still referenced "updated". Align scaffold-dcp.md, design.md, and proposal.md to match the implementation.
