fix(render): post media above its quote; View on X in each card header (v1.6.12)

Two layout bugs the owner hit on the hibtc37 quote-chain capsule.

#88: a post's own images rendered after all the quotes. applySyndicationDataToSegment spliced recovered media and link cards at the end of the segment, which is after the quote card. Rebuilt quote cards still go last; everything else now goes above the post's first quote, matching X's text, media, card, quote order. Regression test in test/dom.test.mjs.

#89: six identical "View on X" links stacked at the bottom of a deep chain. Each card's time and permalink were a footer rendered after its nested quotes, so the links came out innermost first with nothing tying them to a card. They now sit in a meta row directly under each card's author line (View on X ↗ · time), so each link is next to the handle it belongs to. (The first commit message says the link names the handle; it does not, since the author line right above already shows it. The CHANGELOG is correct.) test/quote-chain.test.mjs asserts the permalinks read outermost first in document order.

Second commit bumps to v1.6.12 so Tampermonkey picks it up.

Verification so far:

• npm test, lint and format check pass locally.
• Seen in a screenshot: real syndication data for the 7-post chain rendered at 1280 (dark) and 375 (light). Images sit above the quote, each card carries its own link under the author, and the 375 page has no horizontal scroll (scrollWidth 375).
• Not yet live-verified through a real export; that follows the merge.
