Claude sessions with customTitle metadata displayed generic UUID labels, and forks with copied parent history could disappear because the picker assigned them the parent's session ID.

Read explicit Claude titles before automatic titles, prefer metadata belonging to the current session, and use native UUID filenames to keep fork identities stable. Filter injected prompt blocks individually so a worktree reminder does not hide the human request. Apply title and identity resolution consistently to previews and full imports, including the fork's working directory.

Validation:

• Verified the local parent and fork both appear with their native titles and usable human prompts.
• All 37 core tests passed using a standalone Swift Testing runner linked against the built core module. The normal package test command is blocked by local toolchain framework/macro discovery errors.
• Both new regression tests fail against the original core library and pass against the fix.
• git diff --check passed.
