What does this PR do?

The upstream Claude Agent SDK tree stays on disk (packages/core/src/plugin/provider/claude-agent/** and its tests) so a later sync does not have to resurrect a deleted tree. It is not a product provider. Anthropic stays @shuvcode/claude-plugin (ClaudePlugin).

• ProviderPlugins no longer imports or lists ClaudeAgentPlugin, and drops plugin id opencode.provider.claude-agent if a merge puts it back.
• The exported ClaudeAgentPlugin is a no-op. The on-disk tests still call make.
• The app connect setup dialog, popular lists, settings note, and Claude Agent copy are gone. Catalog helpers drop id claude-agent from integrations, providers, and voice model lists.
• The TUI connect menu and model picker, and CLI auth login/list, drop the same id.
• AGENTS.md and PLAN-fresh-v2-rewrite.md tell future merges not to register it again.

The optional @anthropic-ai/claude-agent-sdk peer and #claude-agent-sdk alias stay so the dormant source still typechecks. They are not wired to a selectable provider.

How did you verify your code works?

• bun typecheck in packages/core, packages/app, packages/tui, and packages/cli. The pre-push hook typechecked the workspace: 40 tasks successful.
• packages/core/test/plugin/bundled-subscriptions.test.ts: ClaudePlugin is in ProviderPlugins and still registers Anthropic claude-pro-max. Plugin id opencode.provider.claude-agent is absent. Running the product ClaudeAgentPlugin export does not register an integration or a provider.
• On-disk packages/core/test/plugin/provider/claude-agent/plugin.test.ts still passes (19 tests) through make.
• App order.test.ts, TUI integration-options.test.ts, CLI auth-login-choices.test.ts, and voice/tools.test.ts pass.
• Did not click through a running app. Connect, settings, and the model picker render the filtered catalogs those tests cover.
