docs: one true story — v2 is live on Base Sepolia - #60

Aligns the docs with on-chain reality (v2 multiplicative + three-tier is deployed and activated) and fixes stale references flagged by the external review: - CHANGELOG: 'v2 not deployed / testnet runs v1' -> v2 deployed + activated, no tasks yet, formula validated in simulation not production. - CLAUDE.md: correct the 'v1 deployed vs v2 spec (not deployed)' orientation to 'v2 is live'; note the audited fixes are on main but not yet redeployed. - README: PRD-04 status -> Complete; 'Smart contracts (v1)' -> (v2); test count 339 -> 408. - Test count 375 -> 408 (deployment runbook). - Stale addresses/URLs: pipeline README Core 0xB655 -> 0x9917 (v2); subgraph docs startBlock 17741070 -> 45036020; cite URLs MarouaBoud -> swarmproof (README, whitepaper, arXiv bundle). Marks P0 (CI #56, license #54, ABI #55/#57, demo numbers #55/#59, false claims #58) and P1 (one-true-story #60, whitepaper Implementation Status #61) complete, and the #41-#44 housekeeping closed.
