Change and reason

Publishes the security and integrity dossier for this repository: a static review of the request surface, a standards assessment, an adversarial exercise, a delivery plan, and the institutional case for acting on any of it. Documentation only — no source file, route, test or configuration default is changed.

Today a prospective user cannot find out what this system protects and what it does not. SECURITY.md is a policy document (how to report a vulnerability), not a model of what is being protected. This PR supplies the model, with file-and-line evidence for every claim, and states plainly what the review could not do.

Findings, in one line each: the code surface is unusually well defended (twelve controls recorded as sound, several better than commercial products), while four absences — no identity, no audit trail, no encryption at rest, no classification model — account for every serious finding. Three of those absences are required HIPAA implementation specifications (unique user identification, authentication, audit controls), not addressable ones.

What is added

File Contents
docs/security/recon-report.md 17 findings (SEC-01…SEC-17), each with the file, line and reproducing command; the 12 existing controls recorded as sound so later work does not disturb them; GDPR article-by-article; HIPAA specification-by-specification; all 33 TOP SECRET (Tier 3) baseline behaviours classified as application, inherited from the environment, or no software expression; a 20-command evidence log; and an explicit list of what was not run
docs/security/colour-teams.md CyBOK-referenced white/red/blue/purple/green/gold exercise: 12 red scenarios with preconditions, a detection matrix (8 of 10 red scenarios would be invisible today), and the four conflicts between teams with how each resolves
docs/security/ultraplan-secure-private.md The engineering plan: 8 workstreams (W0–W8), 18 revertible PRs, each with files, ledger events, acceptance tests, clauses closed and friction cost; plus the four-rung capability ladder
docs/proposals/security-and-integrity-programme.md The case for acting, written for the people who carry institutional risk (research office, DPO, research integrity, IT security, KEC partner) rather than the ones who run the pipeline
docs/proposals/security-and-integrity-ultraplan.md The remedy schedule: 20 remedies keyed to the findings, 9 independently fundable packages, 3 signable gates that state what may and may not be claimed at each, a 12-question partner answer pack, and the decisions needed to start
docs/security/README.md Index and reading order, split into a decision path and an implementation path
SECURITY.md A short pointer section to the dossier
.gitignore Publishes docs/security/ and docs/proposals/ under the existing documentation allowlist (docs/* is ignored by default)

The governing rule

Every proposed remedy is eliminate → substitute → notify, with no control that pushes work onto the user: zero added user actions across the whole programme, and at most two one-off decisions per study. Confirmation dialogs, per-session passwords and per-export classification dropdowns are explicitly rejected on human-factors grounds in favour of substitutes.

Honesty constraints applied throughout

  • No rung of the ladder above R1 is claimed. TOP SECRET is stated as an accreditation of a system inside an accredited environment, not a property of an application.
  • "GDPR compliant", "HIPAA compliant" and "TOP SECRET ready" are named as claims that would be false today.
  • The dossier states that a green CI run is not a claim that a finding is resolved.

Related work left out: see Scope.

Validation

No tests were run, because nothing executable changed. This PR adds five Markdown files, creates two directories, and adds 15 lines to SECURITY.md.

all in range, except four citations that name routes in other route files
(see note below)

Environment, and therefore what is not claimed: no Julia, R, bun, DuckDB, yq or ijson-jcs binary is available in the review environment, and the checkout was a shallow clone (1 commit). Consequently:

  • no test suite, benchmark or proof check was executed here;
  • no live HTTP request was sent to a running server — all exploit paths in the dossier are reasoned from source and are labelled as such until tests exist (ultraplan W7 turns them into tests);
  • no Git-history secret scan was performed (shallow clone). .github/workflows/secret-scanner.yml scans full history in CI; the dossier asserts that design, not its result;
  • no dependency advisory audit of the Julia, R or npm trees was performed.

The four citation notes, stated rather than hidden: report.jl:119, config.jl:631, cladistics.jl:321 and evidence.jl:696 are cited in the deletion-route inventory and refer to route files of those names under src/server/routes/; the checker resolved them to same-named files elsewhere in the tree. They are correct in context and should be read as src/server/routes/<name>.

The dossier's own §0.3 and §10 record these limits, and §9 gives the 20 commands that reproduce every finding.

Scope

  • No code, no routes, no tests, no config defaults, no lockfiles. The plan in ultraplan-secure-private.md is explicitly not applied.
  • No scientific or statistical change. The invariant adopted for the whole programme is that the same inputs produce byte-identical outputs; nothing in this PR touches that code.
  • No claim of certification, accreditation or compliance, and no attempt to bring higher-classification government guidance into the repository. The Tier 3 material is obtained through an organisation's own security team, with the NCSC consulted where there is a business need.
  • Deliberately left for the owner as decisions, not silently chosen: the target rung for the next release, the DPIA owner, the classification vocabulary (UK GSC versus an institution profile), the programme's outward name, and the wording of the author affiliations in the proposal. These are listed as questions in both docs/proposals documents.
  • Signed commits: this branch's commits are unsigned. I cannot sign in this environment. Upstream requires Verified commits for merge (see notes/commit-signing.md), so the branch will need the owner's signing step before merge; the content is unaffected.
  • .gitignore changes only the documentation allowlist, adding two !docs/.../ lines and updating the adjacent comment. No build output, vendored file or generated artefact is included.

Contribution checks

  • I followed CONTRIBUTING.md.
  • This branch starts at current upstream main and contains no merges from fork main. Rebased onto 8a71af8 (the current main tip) with git rebase --onto origin/main d6e8671 after main was rewritten by fix(governance): make the DEED and Scorecard lanes fork-local (restores both) #293; git rev-list --left-right --count origin/main...HEAD reports 0 3.
  • CI has run and passes. — CI has to run against this documentation-only change; no Julia test result is claimed by me.
  • Existing licensing, citation and acknowledgement text is unchanged. LICENSE, CITATION.cff and the existing SPDX headers are untouched; the five new documents carry CC-BY-SA-4.0 headers matching docs/statistics/ and docs/schemas/.

Read docs/security/README.md first. If a finding here is wrong, the command that proves it wrong belongs in the same PR as the correction.

Arena Agent and others added 3 commits October 26, 2026 07:11
Adds docs/security/: a static review of the request surface, data handling and
deployment assumptions at d6e8671, mapped against GDPR, the HIPAA Security
Rule and the UK Government Security Classifications TOP SECRET (Tier 3)
baseline behaviours.

- recon-report.md: 17 findings (SEC-01..17), each with file-and-line evidence;
  12 existing controls recorded as sound so the plan does not disturb them;
standards matrices (GDPR article by article, HIPAA specification by
specification, 33 TOP SECRET behaviours classified as application,
inherited or no-software-expression); an evidence log of 20 reproducible
greps; and an explicit list of what the review could not run.
- colour-teams.md: white/red/blue/purple/green/gold exercise over CyBOK v1.1
knowledge areas, including where the teams disagree and how each conflict
  resolves under an eliminate/substitute/notify rule.
- ultraplan.md: W0-W8 sequenced into 18 revertible pull requests, with the
acceptance test, ledger events, standards clauses and friction cost of each,
  and a four-rung ladder (OFFICIAL -> OFFICIAL-SENSITIVE -> SECRET-candidate ->
TOP SECRET) so partial delivery stays a truthful claim.
- SECURITY.md points at the dossier; .gitignore publishes docs/security/ under
  the existing documentation allowlist.

No source file is changed and no test is claimed: the review environment has no
Julia, R or bun, and the clone is shallow. The dossier says so in its own
limitations section.

…itutional and commercial use

Adds docs/proposals/security-and-integrity-programme.md in the form this project
uses for proposals, written for the audience that carries institutional risk
rather than the one that runs the pipeline.

The proposal argues that the field reviews research software at the level of
detail its authors care about - estimators, parity tolerances, dispersion
priors - and not at the level an institution needs afterwards: who could have
altered a result, who can delete a study, what protects the data at rest, and
what a released artefact says about itself. It uses this repository's own
evidence for the asymmetry: twelve controls that are better than most commercial
products (path containment, an Origin allowlist with DNS-rebinding tests, an R
formula AST, verified reference data, content-addressed analyses, pinned and
least-privileged CI) alongside four absences (no identity, no audit, no
encryption at rest, no classification) from which every serious finding follows.

It records the design provenance - Joshua Jewell, a leading Royal Veterinary
College ethicist and welfare specialist, formerly of the BIOCEV laboratory in
Prague with Charles University, an associate of Vladimír Hampl - and why that
habit produced immutable, hashed, checksum-verified outputs before it produced
identity and audit: ethics and welfare review is the discipline of asking who
bears the risk of a decision, who cannot consent to it, and what evidence will
survive scrutiny.

It also carries a risk table in the terms each owner uses (research office, DPO,
research integrity, IT security, KEC partner), the four-rung ladder so that a
partial delivery is still a truthful claim, the friction budget (zero added user
actions), out-of-scope items, and six questions for the owner.

Renames docs/security/ultraplan.md to ultraplan-secure-private.md and says why:
'secure' is the confidentiality, integrity and availability work, 'private' is
the data-protection work, and they are one programme because they share the
choke point where an artefact is created, labelled, recorded and released.
Updates every cross-reference, publishes docs/proposals/ under the existing
documentation allowlist, and links the dossier index both ways.

… documents

Adds docs/proposals/security-and-integrity-ultraplan.md: the proposal converted
into a remedy schedule. Where the secure-and-private ultraplan answers 'how do we
build it', this answers 'what harm is removed, for whom, what will we be able to
prove, and who signs for it'.

Contents:
- A twenty-entry remedy register. R-01..R-17 close the recon report's findings,
R-18..R-20 are the institutional obligations no code can discharge (DPIA and
record of processing, briefing records, transfer position and sub-processor
register) and that would otherwise be quietly dropped. Each row carries the
harm in the words an institution would use, the causing finding, the
disposition (eliminate, substitute, notify, transfer, or accept-recorded),
the remedy, the earliest gate, and the evidence artefact.
- Nine remedy packages RP-0..RP-8, independently fundable and abandonable, each
with the harms it closes, the engineering workstream it draws on, its owner
(PI, RO, DPO, RI, ITS, KEC), its exit test and the decision it needs.
- Three signable gates plus an explicit refusal of a fourth. Each gate states
what may be claimed and, in the same table, what may not — the pressure to
over-claim during a KEC negotiation is the risk this table exists for.
Gate withdrawal is defined and must happen in the pull request that weakens
  the control it depended on.
- A twelve-question partner answer pack giving the permitted answer at each
rung, because five of the twelve are the ones that stop agreements.
- A KEC and commercial annex covering licence (AGPLv3 stated rather than
discovered), controller/processor position, transfer mechanism, sub-processors,
export control, publication rights, liability and support expectations.
- The register as KYAML data with a CI governance check in the style of
scripts/check-governance-contract.sh, so a remedy cannot be marked verified
without the command that proves it.
- Metrics chosen because they can be wrong visibly, including gate withdrawals:
a gate that never withdraws was never real.
- Stop/go, partial-claim integrity, rollback, the risks of the remedy itself
(compliance theatre, over-claim, scope creep into the science, sole-maintainer
burnout), nine decisions needed to start — three of which a delivery plan never
asks — and five conditions under which this plan is wrong.

Links the remedy plan from the proposal, the dossier index and the engineering
ultraplan, and splits the dossier's reading order into a decision path and an
implementation path.

- Around line 171-176: Update the RP-1 artefacts and exit-test criteria to

include the required RP-6b scrubber and RP-8b briefing and transfer
dependencies; ensure M4 cannot make R3 claimable until these dependencies, as
well as RP-5 and RP-7, are complete.
- Around line 459-461: Update the state rules in Section 6 to support accepted
residual-risk entries, either by adding an accepted state with the owner and
date required by Section 9 or by defining a separate schema for accepted
entries; preserve the existing planned → implemented → verified flow.
- Line 110: Reconcile the R-17 mapping in the register with RP-7 so the same
remedy has one completion point: either separate the initial dossier from the
accreditation pack and map each to its intended rung, or assign R-17 to one rung
and update the package mapping to match.

Review comments at @docs/security/colour-teams.md:
- Around line 190-193: Update the Purple verdict in the ultraplan to match the
table: state that nine of ten red scenarios are not detected today and one is
only partly detected at CI level. Do not claim that two scenarios would be
visible unless they are added to the table.
- Around line 119-122: Update the Red-team summary to match the R-01 through
R-12 results: describe six successful paths, or five closed paths and one
partial path because R-12 has a separate release-artifact gap. Remove the
unsupported claim that ten of twelve paths are closed.
- Around line 89-90: Update the R3 row in the colour-team requirements to remove
the unresolved SEC-30 reference. Use the actual egress findings SEC-05 and
SEC-06, or retain TS-30 alone, while preserving the row’s other requirements.

Review comments at @docs/security/README.md:
- Around line 43-45: Update the W1 summary in the security documentation to
describe a proposed plan rather than completed changes: replace claims that it
removes attack paths, adds protections, and leaves analysis code untouched with
wording that says it is intended to remove them, would add the protections, and
would not touch analysis code.
- Line 12: Update the four route citations in the linked recon-report.md for
doi.jl, config.jl, databases.jl and report.jl to use their full
src/server/routes paths, removing ambiguity with same-named files elsewhere.
- Around line 40-42: Update the TOP SECRET headline in the security README to
identify TOP SECRET as a classification tier and state that accreditation
applies to a system, not an application. Preserve the following statement about
the achievable statement being a rung rather than a classification.

Review comments at @docs/security/recon-report.md:
- Around line 711-714: Update the behaviour count in the matrix introduction
from twenty-eight to thirty-three to match TS-01 through TS-33 and §5.3; keep
the assessment scope unchanged.
- Around line 799-803: Update the C1 heading and summary to describe the single
DELETE request shown after the no-active-job precondition, aligning the request
count with R-01. If a separate setup request is necessary to the finding,
include it explicitly in the chain rather than claiming two requests without
showing one.
- Around line 246-249: Update the identity requirement in the SEC-01
documentation to require a trusted per-request principal for shared-host
authorisation, sourced from the proxy, peer-credentialed IPC, or separate
per-user processes; do not treat the server’s OS account as identifying the
browser user.
- Around line 351-356: Update the SEC-05 evidence and impact text to state that
publication also requires DOI_ENABLED=true and a configured token, and that
DOI_ORIGIN defaults to the Zenodo sandbox. Keep the existing description of the
route’s confirm check and publication flow accurate.
- Around line 764-770: Update the “Reading of the matrix” summary to reflect
that TS-06 is marked non-compliant and TS-31 requires an application
authorisation gate; do not describe TS-28 and TS-30 as the only active
enforcement gaps. Keep the summary aligned with the classifications in the
matrix.

Review comments at @docs/security/ultraplan-secure-private.md:
- Around line 313-320: Clarify the Windows behaviour in the W3.3 startup check
against the R2 requirement: either define and require an attested Windows FDE
check before allowing the gate to pass, or explicitly state that Windows
deployments cannot meet R2. Keep the existing platform-specific checks
unchanged.
- Around line 347-348: Define a single canonical classification value for
`TOP-SECRET` and use it consistently in the machine-readable enum and marking
rules; keep any human-readable display label separate. Apply the same
distinction to the related classification references so string comparisons used
by `effective_classification()` and downstream gates remain consistent.
- Around line 413-418: Update the W5.1 policy so `personal_data: unknown` is
treated as restricted rather than equivalent to `false`; make the ingest refusal
rule explicitly cover `unknown` until review resolves the state, or define a
complete state table that specifies each state’s controls.
- Around line 248-249: Update the DELETE quarantine and audit protocol so a
failed `delete.requested` ledger write cannot leave data moved without an audit
record: define a transaction or compensation sequence that preserves data
recoverability and ledger accuracy. Apply the same failure-safe protocol to the
related flows at the referenced locations and inject failures after each step to
verify both outcomes.
- Around line 366-370: Update the manifest contract in the export workflow to
include copy_number and the total number of copies, and define how both values
are allocated so each export’s machine-readable manifest matches its visible
banner. Apply the same contract consistently to the related manifest
descriptions.
- Around line 641-648: Update the R3 and R4 criteria around W8 so their
sequencing is consistent: make the repository evidence pack produced by W8 a
requirement of R3 and reserve the human accreditation gates for R4, or move the
complete accreditation-pack requirement to R4. Keep the stated W7/W8 ordering
and avoid requiring an output before its producing step.
- Around line 287-289: Update the trusted-proxy authentication flow for
X-Auth-Request-User so loopback access and JULIA_METAMANIFOLD_TRUSTED_PROXY=1
alone do not authenticate the proxy; require a verified proxy-to-application
channel, such as Unix-socket peer credentials or mTLS, before accepting the
header, and add an acceptance test showing a local process without that
authenticated identity cannot impersonate a user.
- Around line 231-243: Update the audit-ledger protocol around AUDIT_EVENTS and
the record shape to define the exact canonical-byte hash pre-image excluding
record_sha256, the genesis prev_sha256 value, and whether newline bytes are
included; specify append and fsync semantics and how an independently anchored
final sequence or signature detects truncation or tail replacement. Include
tests that verify truncation is detected.
- Around line 421-424: Update the W5.2 retention policy so `retention_days`
enforces a maximum grace period: have the `JobQueue` maintenance job move
overdue studies into W2’s two-phase deletion workflow while preserving the
requirement for human action, rather than only recording `retention.due`.
- Around line 171-182: Update the W1.1 CSP rollout guidance to use
Content-Security-Policy-Report-Only for the first release and make enforcement a
separate exit criterion. Resolve the blob: URL concern by documenting whether
the URLs are loaded through a governed directive and adding a source only if
required; remove the worker-script risk reference or identify the file that
creates the worker.
- Around line 210-217: Update the W1.4 egress gate so it validates the
destination before passing an accepted URI to Downloads.download, rejecting
loopback, private, link-local and metadata addresses. Apply the policy to
resolved addresses and every redirect, and prevent DNS rebinding by ensuring the
address used for the request is among those validated; add tests for these
destinations, redirects and rebinding.

Correct the DOI gate description.

The DOI route checks more than confirm=true. It also checks publication enablement, valid study and run names, non-empty files, report-directory containment, and then uses _doi_lock. Describe confirm=true as the irreversible-action confirmation, not as the only gate.

Use the correct classification scope.

This row calls the 33 items “TOP SECRET behaviours”. The stated scope is UK Government Security Classifications Tier 3 baseline behaviours, while §9 says that TOP SECRET guidance is not held or reproduced. Use the Tier 3 wording to avoid contradicting the dossier’s stated exclusion.

Remove the dangling SEC-30 reference.

The recon report defines findings only through SEC-17; it has no SEC-30. Reference the actual egress findings, such as SEC-05 and SEC-06, or reference TS-30 alone. The current R3 requirement is not resolvable.

**Red-team summary.** Ten of twelve attack paths against the *code surface* are
closed and were closed deliberately — this is a well-built application. Every one
of the successful paths is an **architecture** failure, not a coding failure:
there is no identity, so the attacker does not need to defeat anything.

Correct the red-team coverage count.

R-01 through R-06 succeed. R-07 through R-12 fail for their stated attack paths, although R-12 records a separate release-artifact gap. The table therefore supports six closed paths, or five closed paths plus one partial path, not ten of twelve. The current summary materially overstates the security posture.

Reconcile the Purple verdict with its table.

The table marks nine scenarios as No and R-12 as Partly (CI-level). It does not support “two would be visible today” or “eight would not”. State that nine of ten are not detected today and that one is only partly detected, unless the missing two scenarios are added to the table.

Set `0700` on created directories and `0600` on created files
(a `_secure_mkpath`/`_secure_write` pair used by `studies.jl`, `report.jl`,
`figures.jl`, `trees.jl`, `config.jl`). Add a startup check that the data root
is on an encrypted volume where the platform can tell (Linux: `/proc/mounts`
dm-crypt/crypt target; macOS: `diskutil apfs listcryptousers`; Windows: skip and
warn), and record the verdict in the attestation and the startup log. Do not
implement application-level file encryption for the working tree: FDE is the
substitute, and a second layer inside the app adds a key to lose.

Align Windows behaviour with the R2 gate.

W3.3 says Windows skips the FDE check and only warns, but R2 requires an attested FDE volume and restrictive directory modes. A Windows deployment can therefore proceed without the evidence required for R2. Define a Windows equivalent and fail the gate when it is absent, or state that Windows cannot reach R2.

Clarify the Windows behaviour in the W3.3 startup check against the R2
requirement: either define and require an attested Windows FDE check before

Define one canonical classification value.

The machine-readable enum uses TOP-SECRET, while the marking rules use TOP SECRET. If the stored value is compared as a string, classification inheritance, export marking, and publication gating can disagree. Define canonical enum values separately from display labels before implementing effective_classification().

Add the copy number to the manifest contract.

W4.2 promises a copy number on every export, but the listed manifest fields omit copy_number and the total. A machine-readable export can therefore lose the traceability field even when the visible banner contains it. Add these fields to the manifest schema and define their allocation.

Treat unknown as restricted, not as false.

A column-name and sample scan cannot prove that an artefact contains no personal data. If unknown behaves as false, an unrecognised identifier can be accepted without the stricter privacy controls. The refusal rule also names only false, so the default state is ambiguous. Treat unknown as restricted until review resolves it, or define and test a complete state table.

Reword the TOP SECRET headline to separate classification from accreditation.

Line 40 calls TOP SECRET "an accreditation of a system." TOP SECRET is a classification tier. Accreditation is the approval of a system to handle information at a given tier. The intended point is that accreditation applies to systems, not applications. The headline states this loosely, so a reader can take "accreditation" as a property of the tier.

The dossier's assurance scope is not at material risk. The same document already disclaims certification and accreditation elsewhere, so this is a wording correction.

Stage the CSP rollout in report-only mode before enforcement.

The W1.1 snippet sets Content-Security-Policy as an enforced header. The risk table requires one report-only release before enforcement. The W1.1 acceptance criteria name no report-only stage and no later enforcement step. Use Content-Security-Policy-Report-Only for the first release, and make enforcement a separate exit criterion.

The frontend creates blob: URLs with URL.createObjectURL. The policy has no blob: source. Under default-src 'self', those URLs can be blocked when a governed directive loads them, such as img-src. Confirm how each call site loads its URL. Then add the required source, or state why no source is needed.

The risk row also names worker scripts. The frontend search found no new Worker, importScripts, or worker script reference. Remove the worker reference, or name the file that creates the worker.

Add destination filtering to W1.4.

The current gate checks only the URI scheme. Downloads.download receives the accepted URI before sha256 and size are checked, so those checks cannot prevent requests to loopback, private, link-local, or metadata addresses. They also do not provide DNS-rebinding protection.

Define the destination policy and add tests for these addresses, redirects, and DNS rebinding before treating W1.4 as closed.

**W5.2 — Retention.** `retention_days` in config; a maintenance job (reuse the
`JobQueue`) that lists — never deletes — studies past their window, writing
`retention.due` to the ledger. Deletion stays a human act, recorded, and
two-phase from W2.

Make retention_days enforce a data-retention bound.

The maintenance job only lists overdue studies and records retention.due; it never initiates deletion. A study can therefore remain indefinitely beyond retention_days. Add an enforced maximum grace period using W2's two-phase deletion, or rename this setting to a review deadline and remove the bounded-retention claim.

Update the W5.2 retention policy so `retention_days` enforces a maximum grace
period: have the `JobQueue` maintenance job move overdue studies into W2’s
two-phase deletion workflow while preserving the requirement for human action,
rather than only recording `retention.due`.

1. `X-Auth-Request-User` (or a configurable header name) **only** when the
request arrives on loopback *and* `JULIA_METAMANIFOLD_TRUSTED_PROXY=1`. This
is the oauth2-proxy / Authelia pattern: the proxy does the hard part, the app
records the result.

P1 Badge Authenticate the proxy before trusting its identity header

When the proxy forwards to a loopback TCP port, every local process also connects from loopback, so with JULIA_METAMANIFOLD_TRUSTED_PROXY=1 an unprivileged local process can connect directly and submit X-Auth-Request-User: admin. The proposed check would then grant and audit the attacker's chosen identity, leaving the local-peer threat from SEC-01 unresolved. Restrict the backend to a permissioned Unix socket or otherwise authenticate the proxy-to-application channel before accepting this header.

Every route body that takes a `{study}` gets its check through one helper,
`_authorise!(principal, study, role)`, called from `_validate_run_request` and
`_study_exists`'s callers — i.e. from the two functions every route already
calls, so coverage is by construction.

P1 Badge Apply authorization to every study route

The proposed coverage through _validate_run_request and _study_exists misses study-scoped routes that call neither helper. For example, study deletion and rename in src/server/routes/studies.jl:213-240, DOI publication in src/server/routes/doi.jl:58-88, and report reads/writes in src/server/routes/report.jl:50-153 use _study_names or route-specific validators instead. Those operations would remain accessible without the promised per-study ACL, so the R2 attribution/protection claim is not valid unless authorization is enforced centrally or every route is inventoried and wired.

Set `0700` on created directories and `0600` on created files

P1 Badge Enforce restrictive permissions process-wide

Using secure wrappers only in the listed Julia route files does not protect pipeline outputs created by shell tools, R, DuckDB, project initialization, or existing installations whose files already retain the prior umask. On a shared host, those scientifically sensitive files can therefore remain or become world-readable even after W3, contradicting its objective and R2 exit criterion. The plan needs a restrictive process umask plus an upgrade-time permission audit or migration covering both existing and externally generated files.

**W1.2 — Resource caps (SEC-07, SEC-15).**
One middleware that rejects a request whose `Content-Length` (or accumulated
body) exceeds `MAX_REQUEST_BYTES` from `src/analysis/numeric_policy.jl:38` with
`413`; a cap on queued+running jobs in `src/server/jobs.jl` returning `429`; a
cap on SSE subscribers in `events.jl`/`jobs.jl`.

P2 Badge Reject oversized bodies before they are accumulated

For requests sent with chunked transfer encoding or without a trustworthy Content-Length, checking the “accumulated body” in application middleware occurs only after the server has already allocated memory for it, so an arbitrarily large request still achieves the SEC-07 memory-exhaustion attack before returning 413. Enforce the limit while reading at the HTTP transport or reverse-proxy layer, rather than treating a post-buffer middleware check as closing the finding.

**Disposition.** S — a first-class `classification` attribute on study, group and
run, inherited downward, defaulting to `OFFICIAL`, enforced at **one** choke
point on every export path, rendered as a banner in the UI and in generated
HTML/CSV/XLSX/PDF, with a copy number and a dissemination list carried in the
export manifest. Friction: 0 — set once when a study is created; users never see
a prompt, they see a banner.

P1 Badge Fail closed when a study has no classification

Defaulting the new field to OFFICIAL silently assigns the least restrictive tier to every existing study that predates W4 and therefore has no classification. Because the publication gate later explicitly permits OFFICIAL, an upgraded installation can mark and publish legacy sensitive material as releasable without anyone classifying it. Missing classifications should migrate to an unknown or highest-restriction state that blocks export and publication until an authorised owner assigns a tier.

P2 Badge Make the authentication evidence command reproducible

Running this command at the reviewed commit does not produce only the three DOI hits: it also matches session in analysis and R-runtime comments, login throughout validation and remote execution, and password in remote_exec.jl. The authentication conclusion may still be correct, but the dossier's stated rerunnable evidence and W0 acceptance criterion are false as written. Narrow the expression to actual authentication APIs or filter and account for every unrelated match.

**W1.4 — Strengthen the existing egress gates (SEC-06, SEC-05 default).**
`PUT /api/v1/databases`: require `https` for `uri`, require `sha256` (and
`size`) whenever `uri` is present, and require that a change to `uri` carries a
new hash. Keep the advisory warnings, but make the *refusals* hard.

**Acceptance.** Tests: `http://` URI → 400; `https://` with no hash → 400;
`local:` path outside the configured database directory → 400; a legitimate
document round-trips byte-identically.

P1 Badge Pin database provenance rather than accepting a supplied hash

An unauthenticated client can replace a database URI and supply the matching sha256 and size in the same PUT, so every W1.4 validation succeeds for an attacker-controlled HTTPS asset. The checksum proves only that the downloaded bytes match what the attacker just declared; it does not prevent the R-03 reference-database substitution that this workstream claims to close. Pin allowed database manifests or signatures to a trusted source, or defer the closure claim until authenticated, authorised changes cannot introduce arbitrary provenance.

**W2.1 — `src/core/audit.jl`.**
An append-only ledger under `projects/.audit/`, one canonical-JSON record per
line, each record carrying `prev_sha256`, so the chain is verifiable. Reuse
`src/canonical.jl` (`jcs_bytes`) and `SHA.sha256`. Fixed event vocabulary in one
place (`const AUDIT_EVENTS`) so later workstreams extend it deliberately.

P1 Badge Anchor the audit chain outside its writable directory

A prev_sha256 chain stored entirely under projects/.audit/ does not detect an insider or compromised server process that truncates the tail or rewrites a record and recomputes all following hashes; the proposed verifier will accept the resulting internally consistent chain. The acceptance test only detects changing record k without recomputing k+1, so it does not establish the tamper evidence later used for deletion and disclosure claims. Periodically sign or externally anchor the chain head, or write it to independently controlled append-only storage.

**W4.2 — One export choke point.**
Introduce `_publish_artefact(study; bytes, kind, filename, format)` in a new
`src/server/routes/publication.jl`, used by the report ZIP, the XLSX export,
publication tables, figure rasters, tree files and the attestation. It:

P1 Badge Route every downloadable artefact through the choke point

The listed choke-point callers omit existing download surfaces such as the claims YAML attachment in src/server/routes/evidence.jl:638-648, FASTA downloads in src/server/routes/phylogeny.jl:166-170, and arbitrary run outputs served by _file_middleware in src/server/server.jl:400-418. Those responses can therefore leave without classification, copy number, manifest, or ledger event even after W4, invalidating the objective and R3 claim that every export is marked. Inventory all attachment and file-serving routes and enforce publication centrally rather than only in the six named exporters.

Draws on W3.3.

**Artefacts.** Secure creation modes; encryption-position check at start (Linux
dm-crypt, macOS APFS crypto users, Windows warns and records); envelope
encryption for exports and backups.

**Exit test.** A created directory is `0700`; the startup log and attestation
carry the FDE verdict; an export decrypts with the recipient key and not without.

P1 Badge Add envelope encryption to the engineering workstream

RP-4 says it draws on W3.3 and requires an export to decrypt only with the recipient key, but W3.3 implements only creation modes and an FDE-position check; W4 marks exports, and no later workstream encrypts exports or backups. Consequently R-06 remains open and the RP-4/R2 exit test has no implementation PR or acceptance test behind it. Add the envelope-encryption mechanism, recipient-key handling, and ciphertext-hash tests to a concrete engineering workstream before assigning this remedy to R2.

…m to the grid (#296)

## Change and reason

The Hypatia verdict grid on every PR here lists seven enrolled
gitbot-fleet members, all 🟠 *"enrolled, but no report-back was
received"* (see the comment on #295). Nothing in the estate produced the
envelopes the grid reads. The producer now exists in
[hyperpolymath/gitbot-fleet#626](hyperpolymath/gitbot-fleet#626);
this PR wires it into the fork's governance lane.

- `hypatia-scan.yml`: a `fleet` job calling gitbot-fleet's
`fleet-report-back-reusable.yml` at an immutable commit (same owner, so
this repository's Actions policy allows it). `hypatia` now needs both
the contract and the fleet job, runs under `always()` so a dead fleet
job renders as orange rows rather than a missing lane, and still never
runs after a failed contract.
- `hypatia-scan-reusable.yml` (fork-local): **fork delta 4**, the
`report-backs-artifact` input and a tolerant download step that places
the envelopes in `.gitbot-fleet/report-backs/` before "Render the
verdict grid". Upstream standards carries the same change after this
copy's source pin (01c9b0c9); the header records this.
- `scripts/check-governance-contract.sh` guards the wiring: producer
pinned to a 40-hex gitbot-fleet commit, `hypatia` needs
`[workflow-contract, fleet]`, download step present,
`continue-on-error`, and ordered before the render step.
- `.gitbot-fleet/report-backs/README.md` explains how the files arrive
and that none is ever committed.

## Validation

Run locally on this tree (commit 07fe804) with the real adapter binaries
built from gitbot-fleet 68858e09 and rendered through standards
`scripts/verdict-grid.sh` at 9ae2387:

| Reporter | Before | Expected after |
|---|---|---|
| rhodibot | 🟠 indeterminate | 🔴 high (18) |
| panicbot | 🟠 indeterminate | 🟡 medium (6) |
| echidnabot | 🟠 indeterminate | 🟠 not_run, *no ECHIDNA engine endpoint
configured* (now states why) |
| seambot | 🟠 indeterminate | 🔴 high (1: no seam register) |
| finishingbot | 🟠 indeterminate | 🔴 high (14) |
| accessibilitybot | 🟠 indeterminate | 🔴 high (113) |
| glambot | 🟠 indeterminate | 🟡 medium (5) |

All fleet rows are advisory; none sets the lane state.

```
bash scripts/check-governance-contract.sh        # governance contract: all 90 checks passed
bash scripts/tests/test-governance-contract.sh   # 32 mutations rejected, incl. 5 new fleet-* mutations
```

Not run: the workflows themselves (this PR's own CI is the first live
run). `actionlint` was not available in the authoring environment; YAML
was parse-checked with `yq`.

## Scope

- No change to upstream code or CI; fork-only files.
- Depends on gitbot-fleet#626 for the pinned commit to exist on a
branch; bump the pin to the merge commit once it lands (the contract
check only requires an immutable 40-hex SHA).
- No lockfile in this repository.
- Left out: the deed already declares `(fleet :enrolment … :report-backs
…)`; the enrolment manifest is unchanged.

## Contribution checks

- [x] I followed [CONTRIBUTING.md](../CONTRIBUTING.md).
- [x] This branch starts at current upstream main (07fe804) and contains
no merges from fork main.
- [ ] CI has run and passes.
- [x] Existing licensing, citation and acknowledgement text is
unchanged.
